The grout sealant shall spill out of the top of the <br /> well casing to form a mushroom cap seal, with at least 12 inches of neat cement grout <br /> poured 6 inches above and 6 inches below the top of the cut off casing. <br /> The last monitoring event for MW-1 and MW-2 reported results of non-detect for <br /> petroleum hydrocarbons. These wells may be destroyed by pressure grouting from the <br /> bottom upward, while leaving the well casing in place. <br /> • All groundwater displaced to the surface during grout installation shall be pumped out of <br /> the excavation to allow for EHD inspection and verification that the well was properly <br /> destroyed. Contaminated groundwater pumped out of the excavation shall be <br /> containerized for proper disposal. <br /> The Well Contractor shall submit the completed EHD Well and Boring" Permit Application, along <br /> with the correct permit fees of $125 for each well and the $375 Inspection Activity fee to the <br /> EHD. A review of the site records indicates that currently three monitoring wells were installed. <br /> A fee of $750 (3 MW x $125 +$375) will.be required for the well destruction permit.
