SANJ O A Q U I N Environmental Health Department <br /> COUNTY— <br /> Complaint Record Daily Time and Activity Summary from 1/24/2015 to <br /> 8/24/2017 for COTQBWOJS located at <br /> E Liberty and Elliott Rd <br /> Activity Date Recorded by <br /> 05/07/2017 12:00 -Called Cerelle Richards with PGE (925-596-0954)who reported that PE 2546 MANZO <br /> 390 min approximately 60 gallons of transformer oil was spilled from a pole mounted SC 974 <br /> transformer due to a vehicular accident. Cerelle reported that no waterways were <br /> impacted and the spill is mainly on the road way and some on the dirt shoulder. <br /> Cal OES 17-3273. Cerelle indicated that it's on the border with Sacramento <br /> County, she will notify Sac County as well. PGE is being dispatched to DE <br /> energize the lines. The incident occurred less than 30 minutes ago. PSC will be <br /> dispatched to clean up the spill. The driver was intoxicated and hit the power pole. <br /> 12:09 am, received a call back from Cerelle, who indicated that Sac County told <br /> her it's not in their jurisdiction. The ETA for PSC is 2:30 am. According to PGE is <br /> should take approximately 2 hours to clean up and they should be done my 4 am. <br /> Let Cerelle know that I need to witness the cleanup and should be onsite around 4 <br /> am. 12:16 am, received information for the PSC clean up contact onsite: Irene <br /> Pene 831-265-6269, asked Irene to keep me updated on the clean up. <br /> Documented complaint. (30 minutes) <br /> 2:47 am contacted Irene Pene 831-265-6269, asking when she thinks they will be <br /> done with clean up. Irene indicated that they just arrived on scene and they are <br /> waiting for PGE crew to ground the wires move the pole and the transformer. She <br /> indicated that she is hoping she could be done with clean up by 6 or 7 am <br /> depending when PGE is done. I asked to let me know when they are 1.5 hours <br /> away from being done to allow me to get to the site before they demobilize. Let her <br /> know that I plan on being there by 5:30-6 am. Also asked her to let me know if <br /> they are being delayed from the discussed schedule due to PGE work. (18 <br /> minutes) <br /> 3:40 am, received a text message from Irene, who indicated that PGE will be <br /> working on electrical a few more hours and that they hope to get started with the <br /> clean up by 6-7 am and that the site is not ready to do clean up yet. She indicated <br /> that she will give me an update when PSC gains access, she apologized for the <br /> inconvenience of the delay. Based on the information, I will not be heading to the <br /> site yet. (12 min) <br /> 5:44 am, contacted Irene asking about the status of the cleanup. She indicated <br /> that PGE is working on the wires and the pole and PSC doesn't have the access <br /> yet. (12 min) <br /> 7 am, contacted Irene asking about the status of the cleanup. 7:14 am Irene <br /> indicated that PGE is working on the wires and the pole and PSC should gain <br /> access in the next 30 minutes. Asked since its light does she still think that it will <br /> take 3 hours to finish? Received a response back and she indicated that PGE is <br /> being delayed and its another 1 hour instead of 30 minutes. Documented. (24 <br /> min) <br /> 8:36 am, left to go to the site. 8:41 am, let Irene know that that I am on my way. <br /> Onsite. Observed the spill, a large stain on the roadway and impacted soil is <br /> visible on the east and west side of the Elliott Drive, approximately 30 feet from the <br /> Liberty and Elliot intersection. An irrigation ditch is visible on the east side of Elliott <br /> Drive, oil appears to have gone into the ditch. Irene indicated that they will do the <br /> surface clean up today and will USA-dig the site and bring excavator to finish the <br /> soil excavation at ditch area. Asked her to let Cerelle know about the ditch and to <br /> contact the property owner about irrigating. Created complaint and ER <br /> documentation, looked up the property owner for irrigation ditch in CERS, called <br /> the farmer at 209-368-4889, Chris Simonich(CHRIS SIMONICH, 12445 E <br /> LIBERTY RD, GALT, CA 95632, (209) 368-4889), spoke to his wife Robin, her cell <br /> is 209-625-6235. Expressed my concern to Robin regarding the ditch impact and <br /> what might occur if it gets flooded before the spilled is cleaned up, she indicated <br /> that it shouldn't be flooded but she will look into it. Called Cerelle, provided <br /> farmer's contact information and let her know that I had given her number to the <br /> Page 2 of 4 <br />
