INTRODUCTION PERMIT/SERVICES <br /> At the request of Mr. Warren Snider of Western Square Industries,Advanced Geo Environmental, <br /> Inc.(AGE)has prepared the enclosed Site Assessment Work Plan Addendum for the former shallow <br /> disposal well area located at 1565/1621 North Broadway, Stockton, California. The location of the <br /> site is illustrated on Figure 1. A plan of the site is illustrated on Figure 2. <br /> The work plan was prepared as required by the San Joaquin County Public Health Services - <br /> Environmental Health Division(PHS-EHD)by a letter dated 19 June 2000, and in accordance with <br /> USEPA Region 9 Guidelines for Closure of Shallow Disposal Wells - 1992. The scope of work <br /> includes advancement and sampling of one soil boring and collection of ground water samples from <br /> the on-site monitoring wells. <br /> The purpose of the investigation is to determine the lateral and vertical extent of contaminated soil <br /> and/or ground water resulting from the former usage of shallow disposal wells on the property. This <br /> work plan provides procedures for boring advancement, soil sampling, and sample analysis. <br /> A summary of site operations, waste water generation, shallow disposal well abandonment and <br /> sampling and monitoring well installation was included in the following AGE-prepared documents: <br /> Shallow Disposal Well Closure Report, dated 02 November 1999, Preliminary Site Assessment <br /> Work Pleat, dated 19 November 1999 and the Ground Water Monitoring Wells Installation Report- <br /> Fourth Quarter 1999, dated 10 March 2000 <br /> 2.0. SCOPE OF WORK <br /> Based on analytical results of soil and ground water samples collected during monitoring well <br /> installation, additional investigation is required to delineate the vertical and horizontal extent of <br /> selected CAM 17 metals-impact at the site and to verify lack of detectable hydrocarbons in ground <br /> water. Assessment tasks will consist of the following: <br /> • Permitting and pre-field work activities; <br /> • Soil sample collection and analysis; <br /> • Ground water grab sample collection and analysis, if encountered; <br /> • Monitoring and sampling of wells; and <br /> • Report preparation. <br /> Advauced GeoEnvironmental,Inc. <br />
