Multnomah State: OR I Zip: 97283 Phone: 503-240-1919 <br /> PART A <br /> The operator shall attach to this form a written cost estimate in accordance with Part C, in current dollars,of the cost of hiring a <br /> third party to close the major waste Lire facility. Parts Band C shall be based on the maximum quantity of waste tires that the <br /> operator intends to store during the five year permit period as specified in the Operation Plan, Form CalRecycle 501 (6/14). <br /> PART B <br /> The operator shall provide the following information to the CalRecycle prior to commencement of clostue: <br /> 1. A closure schedule with a time period for completion(attach additional pages if necessary): <br /> From a conservative standpoint,25 loads of tires will take 25 days to completely remove the tires from the site.This equals roughly <br /> one load per day,each load consisting of approximately 18.4 tons or 1,600 tires per load. If one were to add a start up day the <br /> Schedule would look like this: Day 1,mobilize;Day 2 load and transport 2 trailers,Day 3 —Day 26 load and transport 1 trailers per <br /> day for disposal. For the 4,000 tons of TDA(352,000 PTEs), 174 loads of TDA will take 35 days to completely remove the <br /> inventory from the site. This equates to roughly 5 loads per day each load weighing approximately 23 tons,or 2,024 PTEs per load. <br /> If one were to add a start up day similar to whole tire removal scenario,the schedule would look like this: Day 1,mobilize,Day 2 <br /> load and transport 5 trailer loads,Da 3-Day 35 load and transport 5 trailer loads per day for disposal. <br /> 2. Details of the final disposition of the waste tires and waste tae products,in accordance with§18441(a). Inchtde the name of <br /> each business that will receive the waste tires and the amounts. Provide the address and phone number for each business <br /> attach additional pages if : <br /> The final disposition of the tires will be: Whole tires will go to: Shamrock Tire Recycling @ 10840 Altamont Pass Rd., <br /> Livermore,CA 209-466-1209,tire shreds to Republic Services Landfill at 4001 N.Vasco Rd.,Livermore,CA 925-447-0491,and the <br /> TDP will go to West Coast Rubber&Recycling at 1501 Lana Way,Hollister,CA 831-634-2800. <br /> 100%of the tires will go to this(or these) permitted facilities for disposal and reuse. <br /> The tires chips will be used to manufacture crumb rubber for use in asphalt,playing fields,and molded rubber products,and/or for. <br /> TDA Type A or Type B,and/or for TDF with similar specifications to TDA. Republic Services at Vasco Rd is the primary <br /> Designate for shreds. The landfills will use the TDA type A and type B as either daily cover(ADC)or gas well development. <br /> 3. A description of how tate closure requirements of§18441 will be met(attach additional pages if necessary): <br /> Public access to the facility will be closed via locking the gates to this fenced and secured facility. A notice will be posted on the <br /> gate(s)informing the public that the site is closed and that the nearest site for drop off will be San Joaquin County transfer station. <br /> The tires and tire chips will be hauled to the destination facilities noted in item 2 which meets the minimum requirements in that each <br /> site is permitted to appropriately receive material. <br /> 1 <br />
