Environmental Health Department <br /> �AN�JOAQUIN <br /> —COUNTY— <br /> Complaint Record Daily Time and Activity Summary from 1/3/2017 to <br /> 8/3/2017 for C00042856 located at <br /> 2115 S SINCLAIR AVE <br /> Recorded by <br /> Activity Date <br /> 01/29/2017 At 06:10pm I was notified by SJGH of a diesel spill at 2115 S Sinclair Avenue. PE 2546 SAEED <br /> 324 min Stockton 95212. Debby told me that two trucks struck each other's causing the SC 944 <br /> spill of 35 gallons of diesel out of one the trucks saddle tank; small amount went to <br /> the storm drain. I asked Debby for the RP contact info, and I was given the State <br /> Warning Center phone number. I called the State warning Center confirmed the <br /> information I received and I was given Erin Key phone number(812) 598-1884 <br /> with Premium Environmental. <br /> At 06:20pm I called Erin to follow up on these information. Erin confirmed the <br /> information I received, and told me that Stockton Fire are on scene, and they <br /> contained the release. I was told by Erin that the incident happened at Sears <br /> Logistics parking lot, and the owner of both trucks are Knight Transportation. Also, <br /> he told me that he hired Patriot Environmental to do that the cleanup and they are <br /> en-route to clean-up the spill. I was given Dave Polloso with Patriot Environmental <br /> phone number( 510) 965-3709. At 06:30pm I called Muni and informed him about <br /> the incident. At 06:41 pm, SJGH paged me again and asked me to call Stockton <br /> Fire. I was given Captain Lindsey phone number, who was on scene. At 06:51 pm I <br /> called Captain Lindsey and told him that I am on my way to the incident. At <br /> 07:15pm I arrived on scene and met with Stockton fire, the trucks' drivers, and the <br /> city of Stockton water division. Zachary with City of Stockton Water division was <br /> on scene and I asked him If Stiffler knows about the incident, and he told me no, <br /> but he will notify him. The spill was contained by Stockton Fire by applying <br /> absorbent around the spill and placing absorbent boom around the storm drain <br /> that is closer to the spill. At 07:30pm I went to the Guard Shack and asked them <br /> to speak to one of the managers of Sears Logistics. I met with Jeff Shanle, <br /> General Manager of Sears Logistics, and Martin Gutierrez, Sears Loss Prevention <br /> Manager. Taking photos was granted by Martin. At 07:45pm I interviewed the <br /> drivers and obtained their information. Both drivers works for Knight <br /> Transportation, and have the same insurance. Stockton Fire left the scene .l <br /> inspected the storm drains One of the Storm drains that are closer to the spill has <br /> small amount of diesel; two of the storm drain that are east of the storm drain has <br /> liquid inside but no sign of sheen, but diesel smell is coming out of them. At <br /> 08:15pm Stiffler and Urbie with the City of Stockton Arrived on scene. At <br /> 09:00pm Patriot Environmental clean-up crew arrived on site, but their supervisor <br /> was not with them. At 09:30pm I issued a notice to abate the driver with punctured <br /> saddle tank, and gave him a copy. This driver name is Hoogstad Eric Wayne. At <br /> 10:05pm I called Erin and asked him about his plan for waste disposal. He told me <br /> that Patriot Environmental will take care of the waste disposal. Patriot <br /> Environmental did not have a plug for the storm drain. Stiffler called Dave and told <br /> him to bring a plug and a Jetter to pressure wash the storm drain. Dave told us <br /> they should be on site within hour and half. Patriot Environmental started <br /> cleaning the contaminated asphalt. At 10:30pm Stockton Fire were call back by <br /> Stiffler to come back to the scene to plug the saddle tank so it does not drip when <br /> the truck is towed. Stockton fire could not seal plug the crack on the tank. They <br /> made a bigger hole so the contractor can pump out the liquid. About 15 gallons of <br /> diesel were pump out of the tank. At 11:30pm Stockton Fire left the site. Martin <br /> Gutierrez asked me if he needs to satay on site, and I told him I recommend that <br /> he stays to approve the final clean-up, but he left the site. <br /> Page 1 of 3 <br />
