Environmental Health Department <br /> SAN.JOAQUIN <br /> —COUNTY— <br /> Complaint Record Daily Time and Activity Summary from 1/3/2017 to <br /> 8/3/2017 for 000042856 located at <br /> 2115 S SINCLAIR AVE <br /> Recorded by <br /> Activity Date <br /> 01/30/2017 At 12:00am Dave arrived on scene. Dave told us that the crew he has now cannot PE 2546 SAEED <br /> 390 min enter the confine space to plug the drain, and we have to wait for another person SC 944 <br /> to do the entry. Three 55 gallons drums of contaminated absorbent were <br /> generated from cleaning up the asphalt at this point. At 12:45am I called Erin and <br /> told him we have been waiting for someone do the confine space entry and Dave <br /> does not have Jetter yet to clean-up the storm drain. Erin told me he will call <br /> Dave. At 12:50am the confine space entry person arrived on site, but no Jetter . <br /> Also United Rental brought Two Plugs to be used to plug the storm drain while <br /> pressure washing. At 03:00am Patriot Environmental could not set-up the plug <br /> inside the storm drain, so they moved to the other storm drain that is farther east <br /> to plug. At 03:30am the storm drain was plugged. <br /> The asphalt was not clean, so I told Dave to clean it more. Dave instructed his <br /> guys to pressure wash the asphalt; and they apply more absorbent to the asphalt <br /> to clean it up. Four 55 gallons drums of contaminated absorbent were generated. <br /> At 04:00am I told Dave to label the drums as"Non-Classified Waste Material'. I <br /> told Dave if he is planning not to label then as Non-RCRA Hazardous Waste, then <br /> we will need the waste analysis from him. He told me that he will discuss this with <br /> his broker, Erin and let us know. At 0600 1 called Muni and updated him on the <br /> clean-up. At 06:07am left the site. At 07:15am StifFler called and told me that <br /> Jetter will be on site at 09:00am. <br /> At 08:00am I came to work and discussed the incident and the plan for follow up <br /> with Muni, Michelle, Garrett, and Kasey. Michelle told me that she can follow up <br /> with this incident today and I can go home and rest. <br /> 01/30/2017 At 08:00am I came to work and discussed the incident and the plan for follow up PE 2546 SAEED <br /> 24 min with Muni, Michelle, Garrett, and Kasey. Michelle told me that she can follow up SC 007 <br /> with this incident today. <br /> 01/31/2017 Michelle Henry told me that another spill of hydraulic oil occurred at the same site PE 2546 SAEED <br /> 150 min due to a release from the Jetter , and clean-up is in progress. She told me to call SC 007 <br /> Brain Uribe today and go back to check on the clean-up status and waste <br /> disposal. I called Brian, but he did not answer. I called Stiffer, and he did not <br /> answer either. At 10:05am I went to the site to follow up with spill. When I arrived <br /> on site I met with Sears EH&S manager Philip Thomas I inspected that cleaned-up <br /> for the spill that happened on 01/29/17. The spill was cleaned-up, and absorbent <br /> pads were placed at the storm drain that is by this area. I took photos of the <br /> cleaned-up area. Philp asked me about the integrity of the asphalt, and I told him <br /> this is my specialty, and I am just here to make sure the waste gets clean-up and <br /> dispose properly. I inspected the hydraulic oil spill, but the affected area looked to <br /> me as the same as yesterday (from the photo that Michelle Henry took yesterday); <br /> I called Michelle Henry and told her that the affected area look the same to me as <br /> her photo. Michelle Henry asked me to call Brian Uribe and ask him if this area <br /> was cleaned up last night or this morning. I called Brian number listed in his card, <br /> but he did not answer. I tried to find a different number for him, but could not find <br /> it. I called Muni and Michelle, and they did not have it. I called Michelle with the <br /> City of Stockton, and asked her to have Brain call me. Brian called me and told <br /> me the contractor cleaned up the hydraulic oil by applying absorbent, and <br /> absorbent were placed in 55 gallon waste drum. I took a photo of the area. I left <br /> the site at 11:45pm <br /> I came back to the office and discussed the result with Michelle Henry. <br /> Page 2 of 3 <br />
