Conduct a complete well survey of domestic, <br /> municipal, agricultural, industrial and any other wells within a 2,000-foot radius of this <br /> site. Contact the Department of Water Resources, municipal water suppliers and nearby <br /> industries for well information. Perform a physical search of the area to identify any <br /> wells. Show the locations of all wells and submit any known construction details of the <br /> wells. Identify all sources of information in the well survey report and clearly depict <br /> each well location as they relate to the above referenced site. Identify any and all <br /> conduits, which may be a potential migration pathway for contaminants from this site. <br /> Provide conclusions on public and environmental health risks as a result of the pumping <br /> influence of these identified wells with respect to the migration and potential impact of <br /> the contaminant plume. <br /> The well receptor survey should be conducted prior to the placement of any new MWs. <br /> If any conduits, such as sewer lines, storm drains, etc., are discovered then it may be <br /> necessary to investigate the potential for contaminant migration along the conduit routes. <br /> If the survey results warrant an investigation of conduits/receptors for possible migrating <br /> contamination from the site then this activity should be proposed as a component of the <br /> new Work Plan. <br /> A Well Survey Reportpermit application $89.00 permit fee City of Stockton <br /> encroachment permit and a Work Plan to further evaluate the lateral extent of the shallow <br /> groundwater contamination at this site should be submitted to PHS/EHD by September 1, <br /> 2000. <br /> Include sampling of the newly installed MWs in the quarterly monitoring schedule for <br /> this site. Continue quarterly groundwater monitoring of all MWs until otherwise directed <br /> by this office.
