Flag City Chevron - 2 - • 25 May 2004 <br /> 6421 Capitol Ave., Lodi <br /> During my phone call, I also reminded AGE that the Monitoring and Reporting Program(MRP) for <br /> C&A 5-00-707 requires an Annual Report,which is overdue. AGE assured me that the Annual Report <br /> would be submitted shortly as an addendum to the Report. <br /> The MRP states: <br /> "An annual report shall be submitted to the Regional Board by 30 April of each year. This report shall <br /> contain an evaluation of the effectiveness and progress of the investigation and remedial actions. The <br /> annual report shall contain the following minimum information: <br /> (a) Groundwater contour maps and contaminant concentration maps containing all data obtained <br /> during the previous year; <br /> (b) Graphical summaries (time-series or trend graphs) of historical data obtained to date; <br /> (c) A discussion of the long-term trends in the concentrations of the pollutants in the groundwater <br /> monitoring wells; <br /> (d) An analysis of whether the contaminant plume is being captured by an extraction system or is <br /> continuing to spread; <br /> (e) A description of all remedial activities conducted during the year, an analysis of their effectiveness in <br /> removing the contaminants, and plans to improve remediation system effectiveness; <br /> (0 The anticipated date for completion of cleanup activities; <br /> (g) An identification of any data gaps and potential def ciencies/redundancies in the monitoring system or <br /> reporting program; <br /> (h) If desired, a proposal and rationale for any revisions to the groundwater sampling plan frequency <br /> and/or list of analytes. " <br /> In order to help evaluate remedial effectiveness and clarify why specific extraction wells should operate, <br /> I request that you include a table of the extraction wells (Table) in future Annual and Quarterly Reports, <br /> which provides: <br /> • The working status of each extraction well pump at that time (e.g., operable, inoperable), <br /> • An explanation how each extraction well is operated (e.g., continuous, alternating, not operated), <br /> and <br /> • An explanation why each extraction well is/is not incorporated into the treatment system (e.g., to <br /> maintain hydraulic control, remediate contamination, or is not needed with a rationale why, etc.). <br /> The Table should also be included with any future letter requests to change the treatment system <br /> operations. I will respond to the Report's recommendation to resume continuous groundwater extraction <br /> from extraction wells EW-7 and EW-10 after I have reviewed the Annual Report. I understand that <br />
