05/04/2800 11:51 2094671118 AGE STOCKTON PAGE 02 <br /> ACCESS AGREEMENT <br /> Monitoring Well Installation <br /> 14749 Thornton Road,Lodi, California <br /> Advanced GeoEnviroumebtal, Inc- (AGE) is hereby gtantod Permission to install one 2-mch <br /> diameter ground water monitoring well at 14749 Thornton Road (APN 055-150-26), Lodi, <br /> California_The investigation is being performed under the directive of the Central Valley Regional <br /> Water Quality Control Board- <br /> The scope of work includes the completion of.one 2-inch diameter ground water monitoring well <br /> utilizing a 9-95 truck mounted drill rig to an approximate depth of 150 feet below site grade(bsg). <br /> The soil boring will constitute a 8-inch diameter borehole.Drilling activities will be completed in <br /> one(1)day. The property owner will be notified at least one 48-!routs prior to performance of field <br /> operations. . <br /> All costs for performance of the subsurface investigation will be the sole responsibility of Flag City <br /> Chevron located at 6421 Capitol Avenue (Mr. Richard Bokides).
