ICS95 <br /> RE: APPLICATION FOR WELL INSTALLATION <br /> SHINKO ELECTRIC AMERICA PROPERTY <br /> 551 CARNEGIE STREET <br /> MANTECA CA <br /> San Joaquin County Public Health Services, Environmental Health Division (PHS/EHD) has received your permit <br /> application for the installation of four monitoring wells at the above noted facility. In order to process this request, <br /> the following additional information is needed: <br /> A workplan describing the construction details for the installation of the wells. <br /> A brief health and safety plan. <br /> A letter from Spectrum Exploration allowing you to sign the permit application an their behalf. <br /> A check for$234.00, the required three hour minimum (@ $78.00 per hour) payment for processing the <br /> request and inspection of well grout placement. <br /> The enclosed "pink" and "green" forms must be completed and signed by the person responsible for the <br /> installation of the wells. If this is a representative of Olin, then a separate letter from the property owner <br /> allowing the wells to be installed at the site is also required. Should staff spend more than the prepaid <br /> three hours on this request, the additional time will be billed to the billing party indicated on the "green" <br /> forth. Please complete this information. The same person responsible for the installation of the wells <br /> must also complete, read and sign the "pink' forth. A signature on this forth acknowledges that additional <br /> billing may be generated and that a copy of all results will also be transmitted to this agency upon <br /> completion of the installation. <br /> A groundwater monitoring plan which describes the frequency of future monitoring and/or sampling from <br /> these wells. PHS/EHD has determined that if wells are not monitored on a regular basis, they tend to fall <br /> into a state of disrepair and may act as a conduit for chemical migration to groundwater. Therefore, if the <br /> wells are not to be monitored on a regular basis, PHS/EHD will request they be destroyed by the person <br /> responsible for the installation. Should this person fail to destroy the wells, PHS/EHD will request the <br /> property owner to destroy the wells. <br /> PHS/EHD will be able to expeditiously process this request upon receipt of the additional information requested.
