Sample labels will be filled out and attached to sample containers at <br /> the time of collection to prevent misidentification of samples. Sample <br /> labels will include the following information: <br /> o Sample identification number. <br /> o Date and time of collection. <br /> o Place of collection. <br /> o Preservation instructions. <br /> o Initials of collector. <br /> a Analyses to be performed. <br /> Samples will be capped and sealed immediately following collection in <br /> order to prevent possible contamination or unauthorized tampering during <br /> transfer. The custody seal will be attached in a manner that requires it <br /> to be broken to open the sample container. <br /> A sample analysis request sheet will be included with all samples <br /> shipped to the laboratory. The request sheet will specify the sample <br /> identification number, the date and time of collection, the type of sample, <br /> the container, the type of analyses requested, and requested analytical <br /> schedule. Instructions for special handling, preservation, storage, or <br /> disposition of the samples will also be included on the request form. <br /> The sample chain-of-custody form will specify the following: <br /> o Sample containers shipped. <br /> o Method of shipment. <br /> o Date shipped. <br /> o Signature of sample collector with time and date of delivery to <br /> shipping company. <br /> o Signature of representative of shipping company who receives <br /> samples from collector or bill of lading signed by shipping company <br /> representative. <br /> A-5 <br />
