The monitoring plan for all four tanks is listed in <br />CERS as 'Monthly Statistical Inventory Reconciliation'. Change this to 'no' in CERS. The monitoring plan must be <br />uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS, make the necessary <br />changes, and submit for review by the EHD. <br />This is a minor violation. <br />110 <br />CCR 2637(e) Failed to submit secondary containment test report within 30 days. <br />Secondary containment testing was performed on June 23, 2015, and a test report was submitted on July 31, 2015, <br />one month and one week late. A copy of the test report must be submitted within 30 days of the tests. Take all <br />necessary precautions to ensure testing and submittal of test reports are performed in a timely manner. <br />This is a minor violation. <br />i 11 <br />CCR 2638(a) Failed to test monitoring and leak detection equipment annually. <br />Annual monitoring system certification and leak detector testing were last performed on March 10, 2015 and was <br />completed on April 11, 2016, one month past due. These tests are required once every 12 months. Take all <br />necessary precautions to ensure testing is performed in a timely manner. <br />This is a repeat violation, Class II. <br />209 <br />CCR 2715(f) Designated operator employee training not performed or log not kept. <br />The designated operator employee training for all employees did not have the HIRE DATE of the employees. This was <br />corrected during the inspection. The designated operator shall train facility employees for which he or she is <br />responsible in the proper operation and maintenance of the UST system once every 12 months. The training shall <br />include, but is not limited to: <br />1. Operation of the UST system in a manner consistent with the facility's best management practices <br />2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br />3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br />4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br />Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of <br />the training records to the EHD. <br />This was corrected on site. <br />This is a minor violation. <br />Page 4 of 5 <br />
