i <br /> Raymond Investment Corporation Page 2 <br /> 730 Channel Street February 1, 2013 <br /> Stockton, Califomia 95202 <br /> directs that 25 pounds per square inch (psi) of liquid grout pressure be applied for a minimum of <br /> ten minutes. Following the pressure grouting, a hole at least one foot larger diameter than the <br /> originally drilled hole must be excavated around the outside of the well casing to a depth of <br /> k three feet bsg. The well casing must then be cut off six inches above the bottom of the <br /> excavation and removed. The grout used to fill the well must then be allowed to spill over into <br /> the excavation and fill the hole for a thickness of one foot and form a cap which has a diameter <br /> at least one foot greater than the diameter of the originally drilled hole. (If augers are used to <br /> excavate the top three feet, the augers must be at least twenty inches in diameter for wells that <br /> were installed using eight-inch augers.) <br /> The remaining wells (MW-2, MW-4, MW-4D, MW-5 and MW-6) have either' impacted <br /> groundwater or are located in an area of known or suspected contamination. These wells must <br /> be over-drilled to total depth and the well contents removed prior to grouting from the bottom <br /> upward to a depth of three to five feet bsg using a tremie pipe. <br /> The EHD approves over-drilling the three soil vapor wells using a four-inch hand auger as <br /> proposed by Condor. <br /> With these conditions the EHD approves the work plan to destroy the wells at this site. Please <br /> submit a copy of the encroachment permit issued by the City of Stockton, the Well and Boring <br /> Permit Application for the well destructions, Master File Record form and appropriate fees for <br /> each parcel. The permit fee for the destruction of each well is $125.00, along with-a $375.00 <br /> Inspection Activity fee per parcel. Since 11 wells currently are installed on two parcels, a fee of <br /> $2,125.00 will be required for the well destruction permits. <br /> Please provide the EHD certification in writing that all record owners of fee title to the above- <br /> referenced site have been notified of your request for closure. This is in accordance with <br /> Section 25297.15 of the California Health and. Safety Code. Submit a complete list of record <br /> owners of fee title and their mailing addresses to the EHD. If you are the sole landowner, <br /> provide a statement to this effect to the EHD.
