(Cambria)for the proposed destruction of monitoring <br /> well MW-16. The well is slated for destruction because it was vandalized and is no <br /> longer functional as a monitoring point for the above referenced site. PHS/EHD agrees <br /> that the well must be properly destroyed. However, before the permit can be issued, <br /> Equiva must also submit the required $60.00 well destruction permit fee, and an access <br /> agreement from the property owner allowing Equiva's contractors entry to the property. <br /> Monitoring well MW-16 is an off-site well that was installed in September 1993. A <br /> license agreement between Texaco Refining and Marketing Inc. and the San Francisco <br /> Baking Company, the business in operation on the property where MW-16 is located, <br /> was submitted to PHS/EHD in July 1993. The license agreement was dated as being <br /> signed in February and March 1993, but specifically states within its body that the grant <br /> of license is effective for five years from the date of August 16, 1989. <br /> Please submit to PHS/EHD the required permit fee and site access agreement. The <br /> current listed property owner of the site where MW-16 is located is Edward Overton, P O <br /> Box 417, French Camp CA 95231.
