San Joaquin County <br /> Environmental Health Department <br /> 1868 East Hazelton Avenue, Stockton, California 95205-6232 <br /> Telephone: (209)468-3420 Fax:(209)468-3433 Web:www.slgov.org/ehd <br /> Large Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> J&E Truck Service and Repair, Inc. 1 1483 W Anderson St, Stockton March 01, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS ll,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 120 HSC 25160.2(b)(4) Consolidated manifest receipts failed to contain complete information. <br /> The following consolidated manifests picked up by American Valley Waste Oil Inc,were found incomplete, missing <br /> the name, address, and identification number of the authorized facility to which the hazardous waste was going to <br /> be transported to. <br /> Invoice 171353,2/20/2019, used antifreeze <br /> Invoice 153775, 9/19/2017, used antifreeze <br /> Invoice 146088,6/14/2016, used oil <br /> All copies of each receipt shall contain all of the following information: <br /> -The name, address, identification number, contact person, and telephone number of the generator,and the <br /> signature of the generator or the generator's representative; <br /> -The date of the shipment; <br /> -The manifest number; <br /> -The volume or quantity of each waste stream received, its California and RCRA waste codes,the waste stream <br /> type listed in subdivision (c),and its proper shipping description, including the hazardous class and United <br /> Nations/North America (UN/NA)identification number, if applicable; <br /> -The name, address, and identification number of the authorized facility to which the hazardous waste will be <br /> transported; <br /> -The transporter's name, address, and identification number; <br /> -The driver's signature; <br /> -A statement, signed by the generator, certifying that the generator has established a program to reduce the <br /> volume or quantity and toxicity of the hazardous waste to the degree, as determined by the generator,to be <br /> economically practicable. <br /> Ensure that all future receipts for hazardous waste shipments using a consolidated manifesting procedure contain <br /> all required information. <br /> This is a minor violation. <br /> 304 CCR 66265.35 Failed to maintain adequate aisle space for unobstructed response to emergencies. <br /> Used oil containers were blocking the entrance to the central hazardous waste accumulation area. <br /> Adequate aisle space shall be maintained to allow the unobstructed movement of personnel,fire protection, spill <br /> control equipment, and decontamination equipment to all areas of facility operation in an emergency. Immediately <br /> maintain aisle space to allow the unobstructed movement of personnel,fire protection, spill control equipment, and <br /> decontamination equipment to any area of facility operation in an emergency. <br /> This is a minor violation. <br /> 602 CCR 66262.34(e)(1)(B) Stored satellite accumulated waste on site longer than one year or 90 days after fill date. <br /> In the shop, one black 55 gallon bucket containing metal fines was observed with a 10/18/2015 accumulation start <br /> date. Satellite accumulated waste shall not be stored on site for more than 90 days after the container was filled, or <br /> a total of one year,whichever comes first.
