<br />ADEQUATE AND NECESSARY ELEMENTS OF THE WORK PLAN [H&S 25299.37,subd(c)(3)]:
<br />1. Destruction of monitoring well MW -1 will be by completely over drilling the well, removing all contents of the
<br />borehole, and filling the borehole with neat cement grout pumped through a tremmie pipe to total depth.
<br />2. Proposed locations of MW -4, MW -5 and MW -6 willbe moved to north/northeast of MW -1. Screen intervals will be:
<br />MW -4, 30-40 feet bsg; MW -5, 55-60 feet bsg; MW -6, 80-85 feet bsg.
<br />3. Installation of two monitoring wells, MW -7 and MW -8, with proposed screen intervals of 80-85 feet bsg.
<br />4. Advancement offsite of seven cone penetrometer test borings to continue the lateral and vertical investigation of the
<br />groundwater contamination.
<br />COME NTS AND CONDITIONS:
<br />1. Verify prior to beginning fieldwork that the proposed locations of MW -5, Mfw-6 and MW -7 will not put them in
<br />pea gravel backfill of the former tank pit.
<br />2. Access agreements from offsite property owners, and encroachment permits for work proposed in the city right-of-
<br />way must be obtained and copies submitted to. SJC/EHD prior to issuance of county well permits.
<br />3. County well permit applications, one per parcel of land, and required $89.00 permit fee must be submitted to
<br />SJC/EHD.
<br />4. A report of findings must be submitted to SJC/EHD within 60 days following completion of the fieldwork.
