The reports are "Site <br />Assessment Report" dated January 18, 2001 and "Site Assessment Work Plan" (work plan) dated March <br />28,2001. <br />The work plan proposes a cone penetrometer test (CPT) investigation followed by a review of CPT data, <br />possible destruction of monitoring wells MW -1 and MW -2, and the installation of up to four pairs of <br />discretely screened monitoring wells. On May 3, 2001, a telephone conversation regarding the <br />investigation took place between Dot Lofstrom of PHS/EHD and Brian Silva and Scott Gable of SECOR. <br />The purpose of the telephone call was to discuss the location of the four pairs of wells. Scott Gable <br />explained that the CPT/monitoring well locations specified in Figure 2 of the work plan were selected with <br />the intent of providing additional lithologic information in order to create a more definitive site conceptual <br />model. The locations were also selected to determine if petroleum hydrocarbon contamination from nearby <br />sites has migrated on to the site at 508 Charter Way. Mr. Gable further explained that the final locations <br />for the monitoring wells would be determined after the CPT investigation is complete and the analytical <br />data have been received. The conversation concluded with an agreement to meet to discuss the site atter <br />the CPT investigation is completed and before the monitoring wells are installed at the site. <br />PHS/EHD hereby approves the work plan to advance CPT boreholes at each of the four corners of the site <br />for lithologic determination as well as groundwater sample collection. Following review of the CPT logs <br />and analytical data, a meeting will be held between PHS/EHD, SECOR, and Chevron (if you choose to <br />attend) to determine the locations of additional monitoring wells.
