The work outlined in this plan has not occurred. <br /> The work as outlined in the approved Work Plan must be completed by March 15, 2000. If the work is not <br /> completed within that time frame the site will not be in compliance with PHS/EHD directives and <br /> reimbursement for corrective action costs from the State Water Resources Control Board Cleanup Fund <br /> may be jeopardized. When the work has been completed, provide PHS/EHD with a report of findings <br /> within 60 days of the work completion date. <br /> In a letter dated July 1, 1999 PHS/EHD requested a well receptor survey to be completed for this site. <br /> PHS/EHD has not received a well survey report. Provide PHS/EHD with a well survey report by March <br /> 15,2000. <br /> PHS/EHD reviewed the Quarterly Monitoring (QM) Report dated December 9, 1999 prepared by <br /> Advanced GeoEnvironmental, Inc. for the above referenced site. Groundwater contamination continues to <br /> be evidenced and appears to be migrating. Continue QM at this site for all groundwater contaminants until <br /> otherwise directed by this office. <br /> A well permit application and $89.00 permit fee should be submitted to PHS/EHD as soon as possible. <br /> Provide PHS/EHD with copies of encroachment permits and off-site boring permits as necessary. After the <br /> permits have been issued by PHS/EHD the work can be scheduled.
