SUPPLEMENTAL FUEL COSTS / ADDITIONAL ITEMS <br /> Supplemental fuel costs (i.e. natural gas, electricity) will be paid direct] }' by Nomellini Construction <br /> Company. However, bidders are required to include ESTIMATES of overall fuel consumption costs, <br /> based upon a 24-month treatment period. Costs for other items, not included in the scope of work, <br /> may be included, if appropriate. <br /> IV. BID REQUIREMENTS <br /> The project must be managed by an appropriately qualified California Registered Geologist with at <br /> least 5 years of experience in UST site investigations and remediation . In addition, the following <br /> items should accompany each bid : <br /> 1 . A total cost for the scope of work, based upon a 24-month treatment period. <br /> 2 . A Statement of Qualifications for the company . <br /> 3 . A list of at least three (3 ) references. <br /> 4 . A list of key personnel who will perform work on the project . <br /> 5 . The consultant' s standard sen7ices agreement. <br /> 6. A completed, signed bid response form. a copy of which is attached hereto. <br /> All personnel performing work on-site must have the appropriate 40-hour OSHA training. <br /> Certificates shall be provided to Nomellini Construction Company upon request. All subcontractors <br /> being utilized for this project must also have any and all appropriate licenses. <br /> All costs for work plans, site health and safety plans, permits, outside contractors, sampling, sample <br /> analysis, supplies, equipment, project management, report preparation and all other associated costs <br /> must be included in a bid for the bid to be deemed responsive. Bids which are judged incomplete <br /> with respect to work effort, costing or technical competency will be considered unresponsive to the <br /> bid specifications and disallowed. <br /> Bids must be complete as proffered. Should a potential respondent require clarification, questions <br /> must be placed in writing and faxed to Mr. Dante John Nomellini at (209) 465-3956. <br />
