J. Hunter & <br /> Associates regarding your client's request for an extension to submit the investigative report for <br /> the site. At our 3 August 1994 meeting, we discussed the need for further investigation at the <br /> facility to define the extent of subsurface contamination caused by the illegal disposal of <br /> hazardous waste into a septic tank and leachfield. At the meeting, we concluded that the <br /> property owners, Mr. William Miles and Ms. Patricia Nolet, would be responsible for <br /> conducting the next phase of investigation which would include looking at surrounding <br /> properties and past on-site operations, surveying for underground tanks, drilling soil borings and <br /> installing ground water monitoring wells. The goal of this work was to define the full extent of <br /> contaminated soil and ground water. A work plan was due by 16 September 1994. <br /> In September 1994, prior to submitting the work plan, the property owners took the initiative to <br /> locate and remove a number of underground storage tanks and associated piping. Extensive soil <br /> contamination found in the vicinity of the tanks and piping was left in place to be addressed at a <br /> later date. Also at that time you submitted a request to extend the submittal date of the work <br /> plan to 16 October 1994; the request was granted. <br /> On 15 November 1994, Wm. J. Hunter & Associates submitted a work plan. The work plan <br /> only proposed one ground water monitoring well which would have been inadequate to define <br /> the extent of contamination. The Board requested that by 6 January 1995 a new work plan be <br /> submitted which would propose at least three monitoring wells located in appropriate locations <br /> to begin defining the extent of contamination related to the underground tanks and the old <br /> leachfield. The 14 February 1995 revised work plan was subsequently approved. The work plan <br /> proposed four monitoring wells and four soil borings at locations appropriate to begin defining <br /> the extent of soil and ground water contamination present throughout the site's subsurface. <br /> Based on the schedule in the work plan, a report was due 1 June 1995. <br />
