<br /> OBSERVATIONS:Many flies throughout the facility. Many dead insects observed behind the large 2 door refrigerators.
<br /> Remove all dead insects and flies. Maintain a pest free facility. The doors were open upon arrival. Keep the doors closed
<br /> and eliminate all gaps on the bottom portions of the rear door and side front door to eliminate all pests.

<br /> OBSERVATIONS:The shelving in the rear of the facility is dirty and uncleanable. Clean the surface and resurface so that it
<br /> can be made cleanable. The buckets with flour, sugar, etc. are not clean or labeled. Clean and label these buckets. Some
<br /> flour was left on the floor in the rear of the facility. Store all ingredients 6 inches off the floor.
