All site employees must be trained prior to taking responsibility as a facility employee if after October 1, <br /> 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall contain <br /> accurate information for all employees trained. Update the Facility Employee Training Certificate to contain the <br /> accurate information and submit a copy of the updated form to the EHD. <br /> This is a minor violation. <br /> 209 CCR 2716(a-e) Failure to conduct the designated UST operator visual inspection at least once every 30 days. <br /> The designated operator failed to check and note all required items on the visual inspection reports. <br /> -Tank tightness, and line tightness test information was left blank on the 11/27/18, 12/19/18, and 3/18/19 inspection <br /> reports. <br /> -The 10/24/18 inspection report left blank the question regarding UDC sensor locations. <br /> -The N answers were not addressed in Section III of the 10/24/18 report. <br /> -The NA answers were not addressed in Section III of the 10/24/18, 11/27/18, 12/19/18, 1/6/19, and 3/18/19 reports. <br /> -Section V was left blank on the 11/27/18 report. <br /> -The question regarding follow up action from the previous report was left blank on the 11/27/18 and 3/18/19 reports. <br /> -The 2/18/19 inspection report incorrectly states overfill prevention testing was completed on time. <br /> During the visual inspection, the designated operator shall check that all required testing and maintenance for the UST <br /> system have been completed and shall check all required items on the report. Ensure that designated operators <br /> performing visual inspections at this facility are including all of the required information on the reports. <br /> A designated operator inspection was performed on 10/24/18 and the next inspection was performed on 11/28/18, 35 <br /> days later. An inspection was conducted on 1/16/19 and the next on 2/18/19, 33 days later. The designated operator <br /> visual inspections shall be conducted within 30 days or less of the previous inspection. Ensure that all designated <br /> operator inspections are performed within the required time frames. <br /> This is a minor violation. <br /> 313 HSC 25290.1, 25290.2, 25291 Failure to construct, operate, and maintain primary containment as product-tight. <br /> Fuel drips were observed on horizontal piping in dispenser 3/4 and dark staining was observed on the conversion frame <br /> beneath the piping, indicating a leak in the primary piping.
