The tank owner is MCM14 Corp, and the tank operator is Parminder Mashiana.The permit holder is Facility Owner. They facility states they are exempt from requirement for an owner/operator agreement.This is not accurate.The annual operating permit is not issued to the current operator and there is no written agreement between the person who was issued the permit and the operator to monitor the UST system. If a permit is issued to a person other than the operator of the tank,that person shall enter into a written agreement with the operator of the tank to monitor the UST system. Immediately log into CERS, upload the written agreement, and submit for review by the EHD or change the permit holder from facility owner to tank operator. Facility personnel changed CERS to state that the permit holder is the tank operator during inspection. This was corrected on site. This is a minor violation.
