The guidelhies specify he constrtctioa and monztariag criteria <br /> n==aryto complate our review of your proposed mdcrground storage tank(CAST) system. <br /> I. SrBML=AL. OF PLANS <br /> IINSTXLI A.TION: <br /> a. Submit 3 complete sets of drawings illustrating the locations of all existing and proposed <br /> structures, including existing UST(s), in non-crasabie print to PHS-lam. Upon approval, <br /> one set will be retained by Pi4S-EM and '-he other two will be returned. <br /> b. Include equipment and material specification sleets and manufacturer's brochures describing <br /> Field testing procedures, operating and monitorin; systems and listing numbers or other <br /> evidence of a aadona-ily recognized testing organization approval. All proposed leak <br /> ,•� detection methods and equipment trust meet the requirements specified in 40 CFR 280.40 <br /> and have been reviewed by the State Water Resources Control Board Stiff. <br /> c, Ala int,-sticial monitors must perform according to manufacturer's standards, and must be <br /> tested annually in accordance with section 2630 (d), California Code of Regulations (CCR), <br /> Title 23 Waters, Division 3.. Chapter 16. <br /> d. Include a detai?ed monitoring and response plan in accordance with se:.don 2632 (d) (1) & <br /> (2). CCR. Title %3 Waters, Division 3, Chapter 16_ <br /> ?. The plans should include a cross sectional diagram of the tonic and piping system including <br /> secondary contaitrmetu, overflow protectioa equipment, and extension of all pipes and/or <br /> ports from below grade to futish grade. <br /> f_ Submit a completed "PHS-•EHD Service Request" form and the State Water Resources <br /> Control Board "Facility Permit (A)" and "Tan's Permit (B)" application forms. <br /> SITE %LITIGATION: <br /> a, In the event contaDcination is observed, confirmed vi <br /> system it is the resporsibiliry of the owner or operator to submit a workplan to PHS-EHD <br /> Site Mt itigation Unit prior to iu:iriating any assessrzent or remediadon activities.
