New facility plan review fee of 3624,00 (S hour minimum per facility), 5170.00 fee per <br /> tank, pias $56.00 state surctrar;e fee per tank is due at time of plan submittal. <br /> �. New installation of an additional tank at an existing facility is $624.00 plan review fee (S <br /> hour minimum per facility) plus $170.00 fee per new tank, and 556.00 per tank state <br /> surcharge fee. <br /> C:. It should be noted that a $624.00 plan review check fee required for each submittal allows <br /> for up to eight (8) hours of staff review' and/or inspection eime_ Additional time spent on <br /> plan review, meetings, inspections, phone or office consultations will be billed to rhe <br /> applicant at a rate of $78.00 per hour. <br /> d. Tie maximum review time for UST installation plans is 40 working days. A prelim nary <br /> review will be conducted by the PHS-EHD within 20 working days of submittal date to <br /> identify any gross plan deficiencies. <br /> e. If gross deficiencies are identified, the plans will be returned with a checklist identifying plan <br /> deficiencies. <br /> f. To expedite the review of installation plans, tune and a half per hour overtime fee may be <br /> submitted to have review completed within 10 worldng days of submittal date if staff is <br /> available to perform the review. <br /> 3. PLAIT REV EW GIUM-EL.ENES <br /> a. The enclosed plan review sheet is to be completed and submitted with the three secs of <br /> drawing-3 to PHS-ETD. <br /> b, A copy of the California UST Regulations may be obtained from the California State Water <br /> Resources Control Board at (916) 227303. <br /> C. Permits and/or plan cheek approval may also be required by the San.foaquin Valley Ur_fled <br /> Air ,Pollution Control District, the County/City Building Department and the Fim District in <br /> your respective area. <br /> d. A minimum of 3 inspections will be conducted on the UST system installation. A $7$ per <br /> hour inspection fee shall be charged for additional impecticas or reinspection as required_ <br /> (See required inspections below) <br /> e. Applicant/corttractor to adhere to the approval plan and any change or deviation shall be <br /> reported to and approved by PHS-EHD prior to insta:lation. <br /> f. The application for installation of underground tanks is only valid for the calendar year in <br /> which it has beer, issued. A permit may be extended into the next calendar year i_f a letter is <br /> sent to PHS-EHD requesti.n,g this extension thirty days prior to the eud of the calendar year.. <br /> A one rime, -tie year extension may bw gramed by PHS-EHD upon receipt of this letter. <br />
