SA N J OA Q U I N AMENDED Environmental Health Department <br /> —COUNTY -,IIZI I <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> FLYING J TRAVEL PLAZA#618` 1501 N JACK TONE RD, RIPON May 08, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item # Remarks <br /> 209 CCR 2716(a-e) Failure to conduct the designated UST operator visual inspection at least once every 30 days. <br /> The designated operator failed to document the following: <br /> On the 10/19/18, 11/15/18, 12/13/18, 1/10/19, 2/7/19, 3/7/19, and 4/4/19 designated operator(DO) inspection <br /> report under <br /> -Section II I <br /> The DO failed to add a description to all the components marked N/A or N from section VII through XI. <br /> On the 5/2/19 designated operator(DO) inspection report under <br /> -Section III <br /> The DO failed to add a description to all the components marked N/A or N from section VII through XI. <br /> -Section IX <br /> The DO failed to check if the under-dispenser containment free of damage, water, debris, and hazardous substance. <br /> The DO failed to mark Yes, No, or N/A to the question "Are all sensors in under-dispenser containment located to <br /> detect a leak at the earliest opportunity?" <br /> Furthermore the designated operator failed to check if L2- Diesel STP 2 fuel alarm on 4/30/19 was responded to <br /> appropriately. <br /> During the designated operator inspection, the designated operator shall review the alarm history for the previous <br /> month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm <br /> history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators <br /> performing inspections at least once every 30 days at this facility and are including all of the required information on <br /> the reports. <br /> This is a repeat violation, Class II. <br /> 306 CCR 2630(d) Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. <br /> The Diesel (tank#1) STP sump sensor was on top of the rib of the saddle of the tank which is approximately 2-3 <br /> inches off the lowest point of the sump. The 91 product (tank#5) STP sump sensor was 4-3 inches off the lowest <br /> point of the sump, the sensor was not raised but closer to the top of the tank saddle. Furthermore UDC 20/21 sump <br /> sensor was on top an electrical piping found in the UDC. The electrical pipe was 2-3 inches from the lowest point of <br /> the UDC. The sensors listed above were not located to detect a leak at the earliest opportunity. Monitoring <br /> equipment shall be maintained to be able to detect a leak at the earliest possible opportunity. The sensors were <br /> relocated to the lowest point of the sump by the service technician.
