The spill <br /> bucket form does not indicate the test method used. Page 2 of the MSC does not have the last three checklist <br /> boxes checked. <br /> The monitoring plan needs a person listed under XI. Personnel Responsibilities - BP West Coast Products, LLC is <br /> not a person. <br /> The response plan needs a person listed under IV.
