Please be advised that the following <br /> issue has been addressed: <br /> Failed to maintain alarm logs and/or records of follow up actions. Maintenance and <br /> monitoring records for the last three years were not found on site. Follow up action <br /> for[a] September 5, 2014 PLLD Open Alarm was not found on site. These records <br /> shall be maintained on site for at least three years. Immediately locate and maintain <br /> all maintenance and monitoring records for the last three years on site and submit <br /> copies to the EHD. Per our conversation this afternoon, the alarm noted in your January <br /> 13, 2015 inspection was received by our remote monitoring contractor, JMM Global, on <br /> September 5, 2014. JMM called the site to investigate the alarm, but the site advised JMM <br /> that one of our maintenance vendors, Gettler-Ryan, was on site, so JMM did not dispatch an <br /> additional vendor. Unfortunately, however, the Gettler-Ryan technician did not note the <br /> alarm in his September 5, 2014 paperwork, and the Designated Operator appears to have <br /> closed the matter after dispatching the alarm with JMM (as it is not mentioned in the October <br /> 2014 DO Report), so there is no documentation of the alarm's final resolution. That said, <br /> the Annual Monitoring System Certification conducted on January 13, 2015 confirmed all <br /> sensor and system functionality, so there should have been no outstanding alarms at that <br /> time. (Copies of the January 13, 2015 Inspection (including the signed Return to <br /> Compliance page) and Monitoring Certification, and September 5, 2014 Gettler-Ryan work <br /> order are attached for your review and reference.) <br />
