The following are Repeat violations: 101, 804, 806 <br /> 101. This facility does not have a valid EPA ID number to manage hazardous waste. A hazardous waste <br /> generator shall not treat, store, dispose of, transport or offer for transportation, hazardous waste without <br /> an EPA ID number. Immediately obtain an EPA ID number to manage hazardous waste and submit <br /> evidence to the EHD by 07/03/2014. A form has been provided. <br /> �0� <br /> 804. One broken Universal Waste fluorescent lamp, and multiple Universal Waste electronic equipment <br /> were not containerized. Universal Wastes, including aerosol cans, lamps, alkaline batteries, mercury <br /> containing switches, CRT, electronic devices, etc. shall be stored in a closed container that prevents <br /> breakage and release to the environment, and the waste containers shall be marked to identify the type <br /> of Universal Waste and demonstrate the initial date of accumulation. Universal waste shall be managed <br /> within one year of storage, and a copy of the disposal receipt for the waste be retained on site for at least <br /> 3 years. Immediately dispose of the above referenced damaged Universal Waste lamp and submit a <br /> copy of the disposal receipt to EHD by 07/03/2014., and provide evidence of how the electronic wastes <br /> are properly managed per this section to EHD by 07/03/2014.
