Failed to label tanks of hazardous waste. One 500 gallon tank of used oil was missing an initial <br /> start accumulation date, A tank of hazardous waste must be property labeled at all times with the <br /> following minimum information: 1. words "Hazardous Waste", 2, contents/ composition, and 3. an initial <br /> start accumulation date. Correct immediately by labeling this tank of used oil with the initial start <br /> accumulation date on the hazardous waste label in the start date column. <br /> # 23. Failed to properly label the container of used oil filters with the initial start date of accumulation. <br /> One 55 gallon black drum of drained crushed used oil filters lacked an initial start accumulation date on <br /> its label. At all times, a container of drained used oil filters must be property labeled with the following <br /> minimum information: 1. words "Drained used oil filters" and 2. an initial start accumulation date. Correct <br /> by having all future drum of used oil filters be properly labeled. <br /> The last disposal receipt stated the facility had their last drum of drained used oil filters picked up <br /> 06/01/09. Drained used oil filters can remain on-site for up to one year of initial start accumulation date. <br /> Correct immediately by having a property licensed hazardous waste hauler removed the drained used <br /> oil filters for proper disposal. Submit a copy of the disposal receipt to SJC EHD within 30 days <br /> (08/27/10). <br /> # 60, 63. Failed to keep designated facility's signed copies of Uniform Hazardous Waste Manifest for a <br /> minimum of 3 years. Missing manifests: 000007747SKS —01/08/2007, 000260595CEX— 03/05/2007 <br /> (under old EPA ID CAL000071133): 000276871 SKS - 04/30/2007, 000543680SKS -- 009/2007 (under <br /> current EPA ID CAL000317937). A generator shall keep a copy of each manifest signed in accordance <br /> with California Code of Regulations, Title 22, section 66262.23(a) for three years or until the generator <br /> receives a signed copy from the designated facility which received the waste. This signed copy shall be <br /> retained as a record for at least three years from date the waste was accepted by the initial transporter. <br /> Correct by submitting missing signed copy manifest or consolidated manifest that is mentioned to SJC <br /> EHD within 30 days (08/27/10). <br /> # 61. Failed to determine status of hazardous waste if not TSDF copy has been received. A generator <br /> who does not receive a copy of the manifest with handwritten signature of the owner/operator of the <br /> designated facility within 35 days of the date the waste was accepted by the initial transporter shall <br /> contact the transporter and/or owner/operator of the designated facility to determine status of the <br /> hazardous waste. Correct by submitting a statement of how this will be conducted. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($115). <br /> THIS FACILITY IS SUBJECT TO REINSPECT.JQN kT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector.TH N � TkI <br />
