REVISED REPORT. <br /> 108. Metal fines generated from the maintenance shop are being recycled as scrap metal without first <br /> determining particle size or hazardous properties. Any person who generates a waste shall determine if <br /> the waste is a hazardous waste. Immediately stop recycling metal fines, make a hazardous waste <br /> determination for the metal fines, and manage it according to Title 22 hazardous waste regulations. <br /> Particles 100 microns or smaller must be handled as hazardous waste if the metal is determined to be a <br /> hazardous waste. Submit a statement and supporting documentation explaining how this waste was <br /> managed by January 18, 2013. <br /> 122 & 125. Copies of hazardous waste disposal records for coolant from 2009, 2010, and 2012, were not <br /> found on site. Hazardous waste generators shall retain copies of all manifests signed off by the disposal <br /> facility and all receipts used in a consolidated manifesting procedure on site for three years and have <br /> them readily available for review. Immediately locate a copy of all manifests and receipts for the last three <br /> years, maintain them on site, and submit copies to the EHD by January 18, 2013. <br /> 126. Evergreen consolidated manifests with work order numbers 564181, 612121, and 606616 lacked <br /> the generator's EPA ID#. All copies of each receipt shall contain all of the following information: (A) The <br /> name, address, identification number, contact person, and telephone number of the generator, and the <br /> signature of the generator or the generator's representative. (B) The date of the shipment. (C) The <br /> manifest number.
