(E) <br /> The name, address, and identification number of the authorized facility to which the hazardous waste will <br /> be transported. (F) The transporter's name, address, and identification number. (G) The driver's <br /> signature. (H) A statement, signed by the generator, certifying that the generator has established a <br /> program to reduce the volume or quantity and toxicity of the hazardous waste to the degree, as <br /> determined by the generator, to be economically practicable. Correct immediately by ensuring that your <br /> own active EPA ID# CAL000352268 and all other required information are used to complete the <br /> consolidated manifests. <br /> 128. Bills of lading for the recycling of used oil filters for 2009 and 2010 were not found on site. A copy of <br /> each bill of lading must be kept on site for at least three years. Immediately locate a copy of all bills of <br /> lading for used oil filters for 2009 and 2010, maintain them on site, and submit copies to the EHD by <br /> January 18, 2013. <br /> 301. There are oil spills on the cement floor next to the two work table outside of the maintenance shop. <br /> Facilities shall be maintained and operated to minimize the possibility of a fire, explosion, or release of <br /> hazardous waste to air, soil, or surface water which could threaten human health or the environment. <br /> Immediately clean up any spilled automotive fluids, and manage according to Title 22 hazardous waste <br /> regulations. Submit a statement and supporting documentation explaining how this waste was managed <br /> by January 13, 2013. <br /> 305. In the oil shed, oil has been spilled into the secondary containment spill control equipment of the oil <br /> tank. The spilled oil is approximately three inches deep in the containment. All facility communications or <br /> alarm systems, fire protection equipment, spill control equipment, and decontamination <br /> where required, shall be tested and maintained as necessary to assure its proper operation equipment, <br /> in time of emergency. Correct immediately by removing the liquid, make a Title 22 hazardous <br /> waste determination, and manage according to Title 22 hazardous waste regulations. Submit a <br /> statement and documentation supporting explaining how this waste was managed by January 18, 2013. <br /> 703. At the time of inspection, it could not be demonstrated (not necessarily documented) that <br /> employees who handle hazardous waste were properly trained. Documentation of employee training for <br /> the facility Spill Prevention Control and Countermeasures Plan, spill clean up, emergency shutoff, fire <br /> prevention, and spill notification indicates that the last training occurred on June 9, 2011. The generator <br /> must ensure that all employees who handle hazardous waste are thoroughly familiar with proper waste <br /> handling and emergency procedures.
