Failed to store Universal Waste lamps in an container that prevents breakage or rebases into the <br /> environment. A single approximately 6 inch fluorescent tube used in transportation buses is found <br /> broken in a small open cardboard box that was sent from the closed facility in Rio Vista. Per shop <br /> manager, these light bulbs are not usable for their facility's buses but could be used in other site's buses. <br /> Any non-usable fluorescent tubes must be placed into a structurally sound container and labeled <br /> properly. Immediately containerize the broken fluorescent tubes in a structurally sound container, <br /> labeled the container as "Universal Waste - fluorescent tubes". Within one year of start accumulation <br /> waste date, have the items properly disposed to an authorized hazardous waste disposal facility. Obtain <br /> a disposal receipt and keep on file for a minimum of 3 years. <br /> 108 & 805. Failed to determine if waste is a hazardous waste and failed to provide training to personnel <br /> handling universal waste. Three Rayovac Ultra Pro size D batteries were found in the garbage can. <br /> Shop manager is unaware that batteries cannot be disposed in the garbage can. Any person who <br /> generates a waste shall determine if the waste is a hazardous waste. Shop mechanics who change <br /> batteries for flashlights or electronic office equipment should be trained on proper disposal, handling and <br /> storage of universal waste. Immediately stop throwing out any types of batteries< s, make a <br /> hazardous waste determination how to properly dispose, handle and store batteries, and manage it <br /> according the Title 22 hazardous waste regulations. Within one year of start accumulation waste date, <br /> have the items properly disposed to an authorized hazardous waste disposal facility. Obtain a disposal <br /> receipt and keep on file for a minimum of 3 years. Submit a statement and supporting documentation, ie <br /> training roster, explaining how alkaline battery waste was managed. <br /> Submit to SJC EHD within 30 days (10/22/12): <br /> 1. one copy of the completed "Return to Compliance Certification" form <br /> 2. a written corrective action statement for the above violations <br /> 3. requested or supporting documentation ie. photos <br /> Notes: <br /> - Used oil, approximately 200 gallons, typically picked up every 2 months <br /> - No body work or paint job performed at facility. <br /> - No interior cleaning/ pressure washing of engine at bus wash. Only bus exterior. <br /> - In 2011 year, facility generated approximately 1300 gallons (5.2 Tons) of used oil and 170 gallons <br /> (0.68 Tons) of used antifreeze = 5.88 tons of hazardous waste. <br /> - Provided: haz waste labeling instruction, free training schedule.
