The monitoring plan is not current and not approved by the EHD. The current monitoring plan for the 87 main, 87 slave, and 91 tanks indicates MLLD Manufacturer as FE Petro, and MLLD Model as STP -MLD. The facility has VMI leak detectors for these tanks. Immediately log into CERS, make the necessary changes, and submit for review by the EHD. This is a repeat violation, Class II. 107 CCR 2632(d)(1)(C) Plot plan/site map not submitted or failed to completely show where monitoring is performed. An accurate UST Monitoring Site Plan was not submitted. A site plan must be submitted identifying the locations where monitoring will be performed. The site map is missing the general piping layout. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/ and upload a copy of the UST Monitoring Site Plan. This is a repeat violation, Class II. 115 CCR 2715(a) Failed to inform the EHD of designated operator identification or change within 30 days. The submitted Owner Statements of Designated Underground Storage Tank Operator Form was incomplete. The form is missing the tank owner identification signature, and date, as well as the ICC certification for Alex Jabbari. Any changes shall be reported to the EHD within 30 days. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov and upload the notification identifying all the designated operators for this facility. This is a Class II violation. 201 CCR 2712(b) Failed to maintain alarm logs and/or records of follow up actions. Maintenance and monitoring records for the last three years were not found on site. The last three years of monitoring system certifications, and secondary containment testing were not on site. Facility has no alarm logs, and the designated operator monthly inspection reports for October 2016 through May 2017 were not on site. These records shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on site and submit copies to the EHD. This is a repeat violation, Class ll. 204 CCR 2712(1) Current monitoring plan approved by the EHD not found on site. An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be accessible on site at all times. Immediately locate and retain a copy on site. This is a repeat violation, Class II.
