<br />Monitoring and response plans not current or approved by the EHD. The monitoring plan is not current and not
<br />approved by the EHD. Monitoring plan information for these tanks lack information. Tank information and monitoring
<br />have applicable sections that have been left blank for tank, vent, vapor recovery, riser/fill piping construction,
<br />mechanical line leak detector, and UDC. Required information in the response plan is blank and or illegible. The
<br />monitoring and response plan must be uploaded to the California Environmental Reporting System (CERS).
<br />Immediately log into CERS, make the necessary changes, and submit for review by the EHD. This is a Class II
<br />violation.

<br />Plot plan/site map not submitted or failed to completely show where monitoring is performed. An accurate UST
<br />Monitoring Site Plan specifying each piece of monitoring equipment was not submitted. A site plan must be submitted
<br />identifying the locations where monitoring will be performed. Immediately log into the California Environmental
<br />Reporting System (CERS) at http://cers.calepa.ca.gov/ and upload a copy of the UST Monitoring Site Plan. This is a
<br />Class II violation.

<br />Failed to submit a written response within 30 days of receiving an inspection report. An inspection was last done on
<br />94-13, 9-28-12, 9-4-12 and inspection reports were issued identifying information to be submitted to bring this site into
<br />compliance. This information was required to be submitted within 30 days of each inspection. This information has not
<br />been received resulting in a non-compliant status for this facility. An operator that receives an inspection report shall
<br />have 30 days to submit a written response that includes a statement documenting corrective actions taken or proposing
<br />corrective actions which will be taken. Ensure that a written response documenting corrective actions taken or
<br />proposed is submitted within 30 days of receiving an inspection report. The 9-4-13 report had violation numbers 106,
<br />108, 109, 201, 202, 310, and 311. The 9-28-12 report had violations numbers 117, 201, 203, 206, 302, 304, 306, 311,
<br />313, and 999. The 9-4-12 report had violation numbers 104, 109, 110, 111, 204, 206, and 303. Refer to the official
<br />inspection reports previously issued to the facility. This is a Class II violation.

<br />Failed to report program data electronically. A submission to the California Electronic Reporting System (CERS) for the
<br />underground storage tank (UST) program is incomplete. Both 1 million" and 1 million" in box A of The Certification of
<br />Financial Responsibility Certification uploaded into CERS should be checked. Only one of these has been checked.
<br />UST Tank Information forms for TA0505568 87, TA0505569 87, TA0505570 diesel, and TA0505571 91 are not current
<br />in CERS. Monitoring plan information for these tanks lack information. Tank information and monitoring have applicable

<br />Compliance information has not been uploaded into CERS. Beginning January 1, 2014, all businesses are required to
<br />submit all new (or any changes to existing) UST information, including: UST Monitoring Site Plan, UST Certification of
<br />Financial Responsibility, UST Response Plan, UST Owner/Operator Written Agreement (if applicable), UST Letter from
<br />Chief Financial Officer, and Owner Statement of Designated UST Operator Compliance online to the CERS at
<br />http://cers.calepa.ca.gov. Be sure to include any other relevant activities and required fields. This is a Class II violation.
