Maintenance and monitoring records for the last three <br />years were not found on site. The maintenance record for the 10-23-13 L8 diesel stp sump fuel alarm was not on site. <br />These records shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all <br />monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual <br />observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other <br />monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and <br />reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on site <br />and submit copies to the EHD. This is a Class II violation. <br />206 <br />Designated operator did not inspect all required items and/or the inspection reports not completed. The designated <br />operator failed to document the response to the attached alarm history on the 10-3-13 designated operator monthly <br />inspection report. The designated operator failed to document that they were responded to appropriately. The missing <br />alarm response documentation includes: the 9-10-13 L19 secondary vapor [vent sump] fuel alarm. During the monthly <br />inspection, the designated operator shall review the alarm history for the previous month, check that each alarm was <br />documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in <br />response to any alarms to the monthly report. Ensure that designated operators performing monthly inspections at this <br />facility are including all of the required information on the reports. This is a Class II violation. <br />314 <br />Primary containment not tight (pre -Jul 2003). The 91 fuel filter in dispenser 1/2 and the diesel fuel filters in dispensers <br />5/6 and 9/10 have a fuel leak. Fuel is collecting in the rim of the fuel filters, indicating a leak in the primary piping. All <br />primary containment for the UST system must be tight. Immediately have a properly licensed, trained, and certified <br />contractor repair or replace the failed component. Depending upon the scope of work, a permit and inspection from the <br />EHD could be required. This is a Class II violation. <br />323 <br />Leak detector failed to detect a 3.0 gph leak at 10 psi. The 91 and auxillary 87 VMI line leak detectors failed to detect a <br />leak when tested. All line leak detectors shall be capable of detecting a 3 -gallon per hour leak at 10 psi. The service <br />technician adjusted the leak detector, retested it, and it passed. This is a repeat violation, Class II. <br />325 <br />Spill container failed to contain a minimum capacity of five gallons. The 91 spill container failed when tested. All spill <br />containers shall have a minimum capacity of five gallons and be capable of containing a spill or overfill until it is detected <br />or cleaned up. Immediately contact a properly licensed, trained, and certified contractor to repair or replace the spill <br />container under permit and inspection of the EHD. This is a repeat violation, Class II. <br />Overall Inspection Comments: <br />On site to witness the annual monitoring system certification testing and conduct the routine inspection. <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by 10-31-14. <br />Consent to perform the inspection, take photos and collect samples was given by Abid Ashrap. This facility's <br />designated operator is Alex Jabbari (ICC expiration date: 10-6-14) .
