Current approved copies of the monitoring and response plans were not found on site. The leak detectors being used are not listed in the monitoring plan. The plan indicates that shutdown occurs at the dispenser even though it does not. The monitoring and response plans must be current and approved by the EHD. Immediately log into CERS, update the monitoring plan information, and submit for review by the EHD. Complete a monitoring plan record for each tank. Immediately submit a copy of the response plan to the EHD for approval. Alternatively, log into CERS, upload the completed form, and submit for review by the EHD. Maintain an approved copy on site. 108. Secondary containment testing was performed on 1-4-13 and a test report was submitted to the EHD on 2-20-13. A copy of the test report must be submitted within 30 days of the test. Ensure that the test reports are submitted to the EHD within 30 days of the test. 109. Annual monitoring system certification, leak detector testing, and spill container testing was performed on 9-28-12 and a test report was submitted to the EHD on 1-10-13. A copy of the test report must be submitted within 30 days of the test. Ensure that the test reports are submitted to the EHD within 30 days of the test. 201. The 9-28-12 monitoring system certification test results and maintenance records for the 2-22-13 sensor alarms were not found on site. These records shall be kept on site for at least 3 years. Immediately locate and maintain the monitoring system certification test results and monitoring system sensor alarm maintenance records on site and submit copies to the EHD by October 4, 2013. 310. Liquid was observed in the diesel turbine sump. If water could enter into the secondary containment by precipitation or infiltration, it must be removed and disposed of properly. Immediately remove this liquid, make a hazardous waste determination per Title 22 hazardous waste regulations, and manage it accordingly. Ensure that the secondary containment sumps are maintained free of liquid - ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
