Acacia, Stockton Program: HW <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I, CLASS II, or MINOR-Notice to Comply) <br /> 812. No disposal records of universal wastes were kept on site. Facility shall retain a copy of each <br /> disposal record of universal waste for at least 3 years on site. Submit a copy of disposal records of <br /> universal waste for the last 3 years to EHD by 02/13/2014, and begin retaining the records per this <br /> section. <br /> NOTES <br /> Unable to determine the amount of hazardous waste generated in the last completed calendar year <br /> (2013) at the facility. It is estimated based on past records and interviews during this inspection that the <br /> facility generates less than 5 tons per year; therefore, the facility remains at the current permitting <br /> category of less than 5 tons. Please be aware that this category may change once EHD has reviewed <br /> all the disposal records to be submitted as a part of your return to compliance requirement. <br /> Facility generates hazardous waste from equipment maintenance and facility maintenance for the <br /> business park. <br /> A copy of CUPA class schedule and San Joaquin County Small Business Hazardous Waste Disposal <br /> Program pamphlet, have been provided to generator during this inspection. <br /> Submit the following to EHD by: 02/13/2014 <br /> 1) A completed copy of the (provided) Return to Compliance Certification (please keep original for <br /> your records). 2) A written statement how each violation was corrected. 3) Evidence (i.e. photos, <br /> disposal receipts, documents) in support of the corrections made. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: Re ived By: Title: <br /> Jeffrey Wong (209) 468-0335 r c , ti14 <br /> EHD 23-02-003 Rev 04/19/12 CONTINUATION FORM <br />
