Mr. Lou Papius <br />ROV: Ad -Art Inc. <br />Page 5 <br />Violation #4: Effective immediately, all manifests shall <br />be correctly and completely filled out. To obtain a State <br />Generator's ID Number, you may contact the Board of <br />Equalization at (916) 322-9070. <br />Violation #5: Within 30 days of receipt of this Report, <br />Ad -Art shall verify receipt of all hazardous waste shipped <br />on Manifest 88294937 by obtaining a copy of the manifest <br />signed by Romic Chemical; or file an Exception Report with <br />the Department. The Exception Report shall be mailed to <br />the attention of Beth Ambacher at Department of Health <br />Services, Toxic Substances Control Program, 10151 Croydon <br />Way, Sacramento, California 95827. <br />Violation #6: If required, Ad -Art shall submit a Biennial <br />Report by March 1, 1990 for the calendar year 1989. <br />Violation #7: Effective immediately, all drums containing <br />hazardous waste shall be marked and labelled with the <br />required wording and information. <br />Violation #8: Within 90 days of receipt of this Report, a <br />personnel training plan shall be prepared in accordance <br />with the requirements outlined in Section 67105, Title 22, <br />Cal. Code Regs. <br />Violation #9: Within 60 days of receipt of this Report, <br />the Contingency Plan shall be updated to include the <br />information required in Section 67141, Title 22, Cal. Code <br />Regs. <br />Violation #10: Effective immediately, all drums <br />containing hazardous waste shall be covered unless it is <br />necessary to add or remove waste. <br />When all violations have been corrected, please send written <br />certification to Beth Ambacher, Department of Health Services, <br />Toxic Substances Control Program, 10151 Croydon Way, <br />Sacramento, California 95827. The certification shall contain <br />the sample certification statement indicated in Attachment 1. <br />The Department will conduct a reinspection of your property to <br />verify compliance. <br />The issuance of this Report of Violation and Schedule of <br />Correction does not preclude the DHS from taking any <br />administrative, civil, or criminal action as a result of the <br />violations noted herein. This matter is being referred to the <br />San Joaquin County District Attorney's office for review. <br />
