An emergency coordinator and modified contingency plan information is lacking. There must be <br />at least one emergency coordinator on site or on call to coordinate emergency response measures, and <br />the following information must be posted by a phone in the maintenance area: the name and phone <br />number of the emergency coordinator; location of fire extinguishers, spill control equipment, and if <br />present, fire alarm; and the phone number of the fire department, unless the facility has a direct alarm. <br />Immediately appoint an emergency coordinator and post the required information by a phone. A form is <br />provided that can be used for this purpose. Submit proof of correction to the EHD by August 25, 2014. <br />804. Approximately twenty to thirty used fluorescent bulbs were observed stored open and not <br />containerized in the maintenance building, at least one of the fluorescent bulbs was broken. Universal <br />waste lamps shall be contained in a closed container or package that is structurally sound, adequate to <br />prevent breakage, and compatible with the contents of the lamp. Immediately containerize all universal <br />waste fluorescent lamps/bulbs in accordance with Title 22 regulations. <br />808. Used fluorescent bulbs were observed unlabeled in the maintenance shop. A generator shall label <br />or mark universal waste with the words 'Universal Waste' and the type of waste (batteries, mercury <br />containing equipment, lamps/bulbs, electronic devices, CRTs, CRT glass, aerosol cans) and an <br />accumulation start date. Immediately label these wastes and ensure that all universal wastes are <br />labeled with the required information. <br />809. Facility was unable to demonstrate the amount of time used fluorescent bulbs were stored on site. <br />A generator of universal waste (batteries, mercury containing equipment, lamps/bulbs, electronic <br />devices, CRTs, CRT glass, aerosol cans) shall accumulate universal waste for no longer than one year <br />from the date the waste was generated. Immediately dispose of this waste as universal waste and <br />submit a copy of the disposal record to the EHD by August 25, 2014. <br />9999. Facility operators are required to upload the hazardous waste generator information into the <br />California Environmental Reporting System (CERS).
