An emergency coordinator and modified contingency plan information is lacking. There must be at least one emergency coordinator on site or on call to coordinate emergency response measures, and the following information must be posted by a phone in the maintenance area: the name and phone number of the emergency coordinator; location of fire extinguishers, spill control equipment, and if present, fire alarm; and the phone number of the fire department, unless the facility has a direct alarm. Immediately appoint an emergency coordinator and post the required information by a phone. A form is provided that can be used for this purpose. Submit proof of correction to the EHD by August 25, 2014. 804. Approximately twenty to thirty used fluorescent bulbs were observed stored open and not containerized in the maintenance building, at least one of the fluorescent bulbs was broken. Universal waste lamps shall be contained in a closed container or package that is structurally sound, adequate to prevent breakage, and compatible with the contents of the lamp. Immediately containerize all universal waste fluorescent lamps/bulbs in accordance with Title 22 regulations. 808. Used fluorescent bulbs were observed unlabeled in the maintenance shop. A generator shall label or mark universal waste with the words 'Universal Waste' and the type of waste (batteries, mercury containing equipment, lamps/bulbs, electronic devices, CRTs, CRT glass, aerosol cans) and an accumulation start date. Immediately label these wastes and ensure that all universal wastes are labeled with the required information. 809. Facility was unable to demonstrate the amount of time used fluorescent bulbs were stored on site. A generator of universal waste (batteries, mercury containing equipment, lamps/bulbs, electronic devices, CRTs, CRT glass, aerosol cans) shall accumulate universal waste for no longer than one year from the date the waste was generated. Immediately dispose of this waste as universal waste and submit a copy of the disposal record to the EHD by August 25, 2014. 9999. Facility operators are required to upload the hazardous waste generator information into the California Environmental Reporting System (CERS). Immediately log into CERS and indicate "yes" to the hazardous waste under the business activities and provide an EPA ID number. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABO D DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. THIS FACILITY IS SUBJECT TO REINSPECTIO IME AT EHD'S CURRENT HOURLY RATE.
