S1 " J o �� ^ � I ` , Environmental Health Department <br /> IH1II,�i_II fl�� N <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> DOLLAR TREE#01263 4555 N PERSHING AVE STOCKTON Aril 11 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS ll,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 106 CCR 66262.34(d)(2) Failed to train employees on waste handling and emergency procedures. <br /> Based on the types of violations observed,the facility employees are not trained on proper hazardous waste(HW) <br /> handling or the training provided has been inadequate. <br /> -A State-Regulated, Toxics, Electronics, OTC Pharmaceuticals, Universal Waste(STOU)HW tote with an <br /> accumulation start date of 4/10/19,was marked"solid"for the physical state, and contained the following(IMG 5353): <br /> -An empty bottle of Assured Instant Hand Sanitizer(IMG 5354-5).When Sandra Rodriguez, assistant store <br /> manager,scanned the item the hand-held scanner prompted her to place the sanitizer into the AF HW tote(IMG <br /> 5356). Sandra placed the hand sanitizer in the AF HW tote. <br /> -The Toon Studio Princess alcohol free hand sanitizer(IMG 5359-60). <br /> - Very Vanilla anti-bacterial hand gel(IMG 5366). <br /> -An empty bottle of Assured Instant Hand Sanitizer(IMG 536405). <br /> According to a document titled"Products To Be Treated As Hazardous Waste"found in the HW storage area, all <br /> hand sanitizers are to be placed in the AF HW tote. Sandra placed the hand sanitizer in the AF HW tote. <br /> -A partially empty bottle of Palmolive dish soap(IMG 5366). <br /> -An empty container of Suavitel fabric softener(IMG 5367). <br /> -A partially empty bottle of April Bath&Shower Lavender&Chamomile Bubble Bath(IMG 5368-9). <br /> -An empty bottle of White Rain Body Wash bagged with a small amount of saturated used absorbent(IMG 5370). <br /> -An empty bottle of LA's Totally Awesome Bang Bathroom&Shower Cleaner(IMG 5371). <br /> -A bag of used absorbent,which appeared fairly dry, and lacked a label as to what item the absorbent was used to <br /> clean up(IMG 5372). <br /> According to Sandra, she did not know what happened to the liquid that was once contained in the empty <br /> containers, but indicated that the empty containers were likely from floor spills or leaks. According to Sandra, floor <br /> spills are cleaned up by using kitty litter to absorb the spill and then placing the used absorbent in a bag with the item <br /> that spilled and into the appropriate HW tote. Only a small amount of used absorbent was observed inside the STOU <br /> HW tote, but not enough to account for the full volume of liquids from the empty containers. <br /> -Sandra pointed out that the hand-held scanner will state Luminessence Scented Candles are"Not Hazardous," <br /> however according to the"Products To Be Treated As Hazardous Waste"document found in the HW storage area, all <br /> candles(including electric and LED)are to be placed in the STOU HW tote(IMG 5374-6, 5378). Sandra stated that <br /> she and the rest of the management team will follow whatever the scanner tells them to do. <br /> -An Aerosols, Flammables and Combustibles(AF)HW tote with accumulation start date 2/3/19 contained an empty <br /> container of Swan Nail Polish Remover. Sandra did not know what happened to the liquid that was once contained in <br /> the empty container, but indicated the nail polish remover may have spilled.There was no absorbent observed inside <br /> the AF HW tote at the time of inspection(IMG 5379, 5381). <br /> -An open black bucket,found on the top shelf of the HW storage area, contained a bag of used paper towels labeled <br /> as hazardous waste, and absorbent that appeared to be unused at the bottom of the bucket(IMG 5382-4). Sandra did <br /> not know who placed the bag of paper towels in the bucket, nor did she know what the paper towels had been used to <br /> clean.
