SHAUNEnvironmental Health Department
COUNTY
—
Small Quantity Hazardous Waste Generator Inspection Report
Facility Name: Facility Address: Date:
DOLLAR TREE 401263 4555 N PERSHING AVE. STOCKTON Aril 11 2019
SUMMARY OF VIOLATIONS
(CLASS 1,CLASS II,or MINOR-Notice to Comply)
Item# Remarks
-A Home Store Lavender scented oil was found in a HW bag on the top shelf of the HW storage area(IMG 5385-7,
5450). Sandra did not know who placed the item in the bag and on the shelf, nor how long the item had been there.
Sandra scanned the item and the hand-held scanner prompted her to place the item in the AF HW tote. Sandra
placed the item into the AF HW tote.
-An Oxidizers, Reactives(OX) HW tote, with accumulation start date 2/1/2019,was broken with a hole in a bottom
corner of the tote(IMG 5388, 5397).The OX HW tote contained the following items(IMG 5390-6):
-LA's Totally Awesome Cleaner With Bleach and absorbent.
-Three containers of LA's Totally Awesome Bleach and absorbent.
-Home Store Bleach and absorbent.
Sandra transfered the above-listed waste and OX HW label to a new tote that was in good condition at the time of
inspection.
-A Corrosive Bases(CB)HW tote, with accumulation start date 2/26/19, contained an empty container of LA's Totally
Awesome All Purpose Concentrated Cleaner with a small amount of saturated absorbent, but not enough to account
for the full volume of liquid from the empty container(IMG 5398-40). Sandra did not know what happened to the liquid
that was once in the container.
-An STOU HW tote, with accumulation start date 3/18/19, contained the following items(IMG 5403):
-A Sunbeam Super Heavy Duty D+battery. The ends of the battery were not wrapped with tape, as indicated on
page 12 of the Dollar Tree California Training Guide on Hazardous Waste Handling and Processing Procedures(IMG
5404-8).
-Three leaking containers of Ajax powder with bleach, each labeled as STOU(IMG 5409-12).When Sandra scanned
the Ajax, the hand-held scanner prompted her to place the Ajax into the STOU HW tote(IMG 5313). Sandra also
scanned a product container of Comet powder cleaner with bleach, which was to be placed in the STOU HW, if
damaged, according to the hand-held scanner(IMG 5414). However, according to the"Products To Be Treated As
Hazardous Waste"document,"Ajax or Comet Type Powdered Cleaner(with bleach)"is to be placed in the OX HW
tote(IMG 5378). Also according to the Dollar Tree California Training Guide and the Potentially Hazardous Material
Management poster(IMG 5351), posted on a wall adjacent the HW storage area, "bleach"is a key or identifying word
to determine an item to be placed in the OX HW tote. Sandra removed the Ajax containers from the STOU HW tote
and placed them into an OX HW tote.
-An STOU HW tote, with accumulation start date 4/10/19, contained the following items(IMG 5415):
-Five leaking containers of Ajax powder with bleach(IMG 5416). Sandra removed the Ajax containers from the STOU
HW tote and placed them into an OX HW tote.
-Five Sunbeam batteries, three of which were in a plastic battery holder. The ends of the batteries were not wrapped
with tape,as indicated on page 12 of the Dollar Tree California Training Guide on Hazardous Waste Handling and
Processing Procedures(IMG 5417-23).
-An STOU HW tote, with accumulation start date 3/3/19, contained the following items(IMG 5424):
-Two containers of Ajax powder with bleach(IMG 5425).
