SAN M Q U I N Environmental Health Department <br /> CC <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> DOLLAR TREE#01263 4555 N PERSHING AVE STOCKTON Aril 11 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 5445-6).A box containing many unopened packs of the cleaner concentrate were found near the store's mop sink in <br /> the back storage area(IMG 5442-44).According to Sandra, the cleaner concentrate is used to clean the floor and <br /> once a pack is emptied the package is thrown in the trash. Meanwhile,the facility is managing emptied containers of <br /> similar types of cleaning products as hazardous waste. <br /> -The following items were found in a trash bag with a yellow sticker marked with the initials"JP"and date 4/10/19: <br /> -A bag labeled as hazardous waste and marked with"STOU"and the initials"JP"(IMG 5429). <br /> -A bag labeled as hazardous waste, and marked with"2942 STOU,"the initials"SB"(or SR), and date 3/17.The <br /> bag contained a clear plastic sphere with an electronic or LED light inside(IMG 5430-1). <br /> -A bag labeled as hazardous waste, and marked with"6826 CB,"the initials"SB"(or SR), and date 2/16. The inside <br /> of the bag was contaminated with a white unknown substance and contained cardboard(IMG 5432). <br /> -A bag labeled as hazardous waste, and marked with an illegible number,"OX,"illegible initials, and date 8/13.The <br /> inside of the bag was contaminated with an unknown liquid (IMG 5433-4). <br /> -A bag labeled as hazardous waste, faintly marked with"STOU,"and marked with illegible initials and date 2/24. <br /> The inside of the bag appeared contaminated with an unknown substance(IMG 5435). <br /> -A bag labeled as hazardous waste and containing a small amount of absorbent.The inside of the bag appeared <br /> contaminated with an unknown substance(IMG 5436). <br /> -A bag labeled as hazardous waste.The inside of the bag appeared contaminated with an unknown substance(IMG <br /> 5437). <br /> -A bag labeled as hazardous waste. The inside of the bag was contaminated with a liquid substance(IMG 5438). <br /> According to Sandra, an employee by the name of Jerry, hence the initials"JP"marked on the yellow sticker, verified <br /> that the items in the trash bag, including the hazardous waste bags listed above,were okay to throw into the trash. <br /> However,there is not an employee with the initials JP, listed on the employee hazardous waste training record, <br /> provided to the inspector by Courtney Tidwell,store manager, at the time of inspection.Also, Sandra did not know <br /> what items were once contained in each of the hazardous waste bags, nor where those items might be. According to <br /> page 21 of the Dollar Tree California Training Guide,the Yellow Dot Trash Program requires store managers and <br /> assistant store managers to visually inspect each trash bag, box, or loose item, prior to it being placed in the <br /> dumpster,to prevent any hazardous waste from ending up in the dumpster(IMG 5447). <br /> -An unlabeled tote was observed in the front of the store adjacent the cash registers and on a bottom shelf beneath <br /> the store's"go-back"items(IMG 5451-2).The unlabeled contained the following items in bags labeled as hazardous <br /> waste(IMG 5453-8): <br /> -White powder and a lid with holes(appeared to be from a container of Ajax or Comet with bleach) <br /> -Two Chap Ice Lip Balms <br /> -A Garden Solar Stake Light <br /> -A Saint Patrick's Necklace Coller containing a light <br /> -Three electric LED tealight candles <br /> -Luminessence Scented Wax Melts <br /> -A Luminessence Metallic LED Votive Candle <br /> -A pack of Fashion(ballpoint) Pens Stylos Mode <br /> -A pack of mechanical lead pencils <br /> -Two broken Sunbeam clear light bulbs <br /> -A wax prayer candle <br /> FA0007925 PR0538093 SCO01 04/11/2019 <br /> EHD 22-01 Rev.09/27/2018 Page 8 of 17 Small Quantity Hazardous Waste Generator OIR <br /> 1868 E.
