Lodi, CA 95242 SR0057987 <br /> The amended permit to break out and remove concrete around all three fill sump man ways, <br /> relocate and plumb the annular riser, replace the gasket/o-ring on the 91 spill container, replace <br /> the 87 annular sensor, and disconnect 4nd reconnect all three annular sensors for the underground <br /> storage tank system (UST) at the above referenced site has been approved with the following <br /> conditions. <br /> Conditions of approval: <br /> 1. Contact the local Fire Department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits. <br /> 2. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br /> a. One of several permissible valid, current contractor's licenses issued by the <br /> Contractors State License Board(may be issued to the installation company), and <br /> b. All valid, current California UST certificates issued by the International Code <br /> Council (ICC), that are applicable to the work being performed, and <br /> c. A valid, current certificate of training from the manufacturer(s)of the <br /> component(s)being tested, repaired, or serviced. <br /> and <br /> d. Each person installing or testing any component must hold a certificate of <br /> training from the manufacturer of that component. <br /> 3. Permit expires 180 days from date of approval. Note: If an extension to the <br /> expiration date is required, a written request must be submitted to this office prior <br /> to the expiration date. <br /> 4. All components must be certified by an independent testing organization. (UL, ULC, <br /> ANSI,API, ASTM, etc. See California Code of Regulations(CCR),Title 23 Chapter 16 <br /> Table B for complete list). The component must bear the appropriate mark as required by <br /> the independent testing organization. <br />
