The following is an itemized list of hazardous waste violations that have not been <br /> addressed for JAAS AUTO SERVICE as of Apri129, 2019. <br /> Open violations from June 16, 2015 inspection <br /> Violation #121 - Failed to retain copy of manifest or bill of lading for spent lead acid batteries for 3 years. <br /> Bills of lading or manifests for the management of lead acid batteries for the last three years were not found on site. <br /> A copy of each bill of lading must be kept on site for at least three years. Immediately locate a copy of all bills of <br /> lading or manifests for lead acid batteries for the last three years, maintain them on site, and submit copies to the <br /> EHD. <br /> Violation#123 -Failed to submit a written response within 30 days of receiving an inspection report. <br /> An inspection was last done on September 24, 2014 and an inspection report was issued identifying information to <br /> be submitted to bring this site into compliance. This information was required to be submitted by October 24, 2014. <br /> This information has not been received resulting in a non-compliant status for this facility. The outstanding <br /> violations include: <br /> CCR 66262.12 Managed a hazardous waste without a valid EPA ID number <br /> CCR 66262.11, 66262.40(c) Failed to determined if a waste is a hazardous waste or retain waste analysis on site <br /> for 3 years <br /> CCR 66265.51 Failed to prepare and implement a contingency plan <br /> CCR 66265.16(a-c) Failed to complete training on hazardous waste management and emergency response <br /> procedures <br /> CCR 66262.40(a) Failed to keep a signed copy of manifests from the designated facility for three years <br /> HSC 25160.2 Failed to follow all consolidated manifesting procedures <br /> CCR 66266.81(a)(4)(B) Failed to retain a copy of manifest or bill of lading for spent lead acid batteries for 3 years <br /> HSC 25189.5(a) Disposed or caused disposal of HW at an unauthorized point <br /> CCR 66265.31 Facility not maintained to minimize the release of a hazardous waste <br /> CCR 66265.173 Failed to keep hazardous waste containers closed except when adding or removing hazardous <br /> waste <br /> CCR 66265.192(a) Failed to comply with new hazardous waste tank system assessment requirements <br /> CCR 66265.195 Failed to conduct daily inspections of hazardous waste tank system and maintain records onsite <br /> CCR 66262.34(f) Failed to completely label containers or tanks of hazardous waste <br /> CCR 66266.130 Failed to properly manage used oil and fuel filters <br /> CCR 66273.33.5 Failed to properly manage universal waste electronic devices, CRTs or CRT glass <br /> HSC 25201.16(e) Failed to manage aerosol cans in a manner that prevents the unauthorized release of waste <br /> HSC 25201.16(g) Failed to store used aerosol cans in an accumulation container <br /> CCR 66273.34 Failed to label universal waste to identify type of universal waste <br /> CCR 66273.36 Failed to train employees and maintain records on UW handling and emergency procedures <br /> CCR 66273.39 Failed to keep complete records of each shipment of universal waste for three years <br /> An operator that receives an inspection report shall have 30 days to submit a written response that includes a <br /> statement documenting corrective actions taken or proposing corrective actions which will be taken. Ensure that a <br /> written response documenting corrective actions taken or proposed is submitted within 30 days of receiving an <br /> inspection report. <br /> Violation #202 -Disposed or caused disposal of HW at an unauthorized point. <br /> A partially full aerosol can of brake cleaner with a broken actuator was observed in the yard behind the shop. Mr. <br /> Bongcaron stated that if aerosol cans cannot be fully used due to damage, they are thrown in the trash. No <br /> container was observed on site for the collection of waste aerosol cans. Hazardous wastes shall be disposed of <br /> only by transportation to a permitted hazardous waste treatment, storage, and disposal facility(TSDF). Immediately <br /> cease disposal of hazardous waste to the trash and ensure that all future waste is hauled by a licensed hazardous <br /> waste transporter to a permitted TSDF. <br /> Page 11 of 17 <br />
