The following is an itemized list of hazardous waste violations that have not been <br /> addressed for JAAS AUTO SERVICE as of Apri129, 2019. <br /> Open violations from August 02, 2016 inspection <br /> Violation#123 -Failed to submit a written response within 30 days of receiving an inspection report. <br /> An inspection was last done on June 16th, 2015 and an inspection report was issued identifying information to be <br /> submitted to bring this site into compliance. This information was required to be submitted by September 8th, 2015. <br /> This information has not been received resulting in a non-compliant status for this facility. <br /> The outstanding violations include: <br /> -CCR 66262.12 - Managed a hazardous waste without a valid state ID number <br /> -CCR 66262.11,6626.40(c) -Failed to determine if a waste is a hazardous waste or retain waste analysis on site for <br /> 3 years <br /> -CCR 66265.51 - Failed to prepare and implement a contingency plan <br /> -CCR 66265.16(a-c) -Failed to complete raining on hazardous waste management and emergency response <br /> procedures <br /> -CCR 66262.40(a) - Failed to keep a signed copy of manifests from the designated facility for three years <br /> -HSC 25160.2 - Failed to follow all consolidated manifesting procedures <br /> -CCR 66266.81(a)(4)(B) - Failed to reain a copy of manifest or bill of lading for spent lead acid batteries for 3 years <br /> -CCR 66265.5(a) - Disposed or caused disposal of HW at an unauthorized point <br /> -CCR 66265.31 - Facility not maintained to minimize the release of ha hazardous waste <br /> -CCR 66265.173 - Failed to keep hazardous waste containers closed except when adding or removing hazardous <br /> waste <br /> -CCR 66265.192(a) - Failed to comply with new hazardous waste tank system assessment requirements <br /> -CCR 66265.195 - Failed to conduct daily inspections of hazardous waste tank system and maintain records on site <br /> -CCR 66262.34(f) - Failed to completely label containers or tanks of hazardous waste <br /> -CCR 66266.130 - Failed to properly manage used oil and fuel filters <br /> -CCR 66273.33.5 - Failed to properly manage universe waste electronic devices, CRTs or CRT glass <br /> -HSC 25201.16(a) - Failed to manage aerosol cans in a manner that prevents the unauthorized release of waste <br /> -HSC 25201.16)g) - Failed to store used aerosol cans in an accumulation container <br /> -CCR 66273.34 Failed to label universal waste to identify type of universal waste <br /> -CCR 66273.39 Failed to keep complete records of universal waste for three years <br /> An operator that receives an inspection report shall have 30 days to submit a written response that includes a <br /> statement documenting corrective actions taken or proposing corrective actions which will be taken. Ensure that a <br /> written response documenting corrective actions taken or proposed is submitted within 30 days of receiving an <br /> inspection report. <br /> Violation #301 -Facility not maintained to minimize the release of a hazardous waste. <br /> Black staining was observed on the asphalt surrounding the used oil tank, used oil barrels, oil filter barrels, shop <br /> area and under the engines stores in the yard behind the shop. Rags contaminated with waste oil and waste <br /> absorbent were observed in the floor of the shop. Facilities shall be maintained and operated to minimize the <br /> possibility of a fire, explosion, or release of hazardous waste to air, soil, or surface water which could threaten <br /> human health or the environment. Immediately clean up waste fluids and remove stains from the asphalt, and <br /> manage according to Title 22 hazardous waste regulations. Submit a statement and supporting documentation <br /> explaining how this waste was managed. <br /> Violation #403 -Failed to keep hazardous waste containers closed except when adding or removing <br /> hazardous waste. <br /> The following containers were open to the environment while not in use: <br /> -One black, plastic, 55 gallon drum of used oil and fuel filters was observed with the lid off <br /> -One blue, metal, 55 gallon drum of used oil and fuel filters was observed with the lid off <br /> -Two white, plastic, 5 gallon buckets of oily rags were observed with the lids off <br /> -Waste oil tank at the rear of the facility was observed with an open bung hole <br /> All hazardous waste containers shall be closed at all times except when adding or removing waste. Immediately <br /> close these containers and ensure all hazardous waste containers are closed when not adding or removing waste. <br /> Page 7 of 17 <br />
