6. Contact Ray von Flue(468-9848)to arrange the following inspections (a 48-hour notice <br /> is required): <br /> a. EHD must witness the functionality of the monitoring system to ensure that <br /> monitoring has been restored to meet the requirements of California Code of <br /> Regulations Title 23 and the Health& Safety Code. <br /> 7. The monitoring must satisfy this facility's approved monitoring program. <br /> 8. An "Underground Storage Tank Certification of Installation/Modification" form shall be <br /> submitted by the owner or the owners'agent [pursuant to (CCR)2635(d)] before this <br /> upgrade will be considered complete by EHD. <br /> 9. Permit expires 180 days from the initial approval date(March 24,2009).
