5) Notifying the PM and SSO of special medical problems and confirming that on- <br /> site personnel are aware of any such problems. <br /> BSK personnel associated with the field operations of the project include: <br /> Project Manager: Richard Johnson <br /> Site Manager/Site Safety Officer: Jeff Yeazell <br /> Regional Health and Safety Manager: Robert Archer <br /> 4.5 Subcontractor's Safety Representative- <br /> Each subcontractor is required to designate a safety representative. That individual is responsible <br /> for the safe and healthful performance of work by,his work force and subcontractors. During the <br /> subcontractors' activities on-site, the safety representative will perform continuing work area <br /> inspections, and conduct safety meetings and safety orientations for new employees. He will <br /> attend periodic safety meetings with the SSO. The subcontractors'representative will also <br /> investigate accidents and over-exposures involving subcontractors'personnel. <br /> 5.0 JOB HAZARD ANALYSIS <br /> Chemical hazards on site include those associated with waste oil. Physical hazards include those <br /> from on-site traffic, drill rig operation, and underground and overhead utilities, if present. <br /> 5.1 Drill Rig Hazards <br /> Safe work practices for drill rig operation include: <br /> • When moving a rig off-road, pay particular attention to obstacles in the route of <br /> travel. Walk the route first. <br /> • Have an assistant guide the driver when in close proximity to hazards or if <br /> clearance is at a minimum. <br /> • Set the brakes and/or.block the wheels when the derrick(mast)is set up. <br /> • The derrick must be lowered when traveling off-road. <br /> • Be certain of the status of utilities (energized, charged, etc.) in the area of the rig. <br /> Call Underground Service Alert at least 48 hours in.advance of performing field <br /> work for assistance in locating utilities. <br /> • Consider overhead lines to be live. Watch for sagging lines when entering a site; <br /> call the specific utility company for assistance in moving them. <br /> • Do not operate a rig within 15 feet of overhead lines; check with local authorities <br /> to confirm the distance requirement. <br /> • Clearing the site of obstructions prior to setting up the rig. <br /> A-4 <br />
