Any changes shall be reported to the EHD <br /> within 30 days. Immediately log into the California Environmental Reporting System (CERS)at <br /> hftp://cers.calepa.ca.gov and upload the notification identifying all the designated operators for this facility. <br /> This is a minor violation. <br /> 201 CCR 2712(b) Failed to maintain alarm logs and/or records of follow up actions. <br /> Maintenance and monitoring records for the last three years were not found on site. The missing records include: <br /> -2014 monitoring certification test results <br /> -2015 monitoring certification test results 00 S14- <br /> -2014 secondary containment test results <br /> -Alarm logs documenting the facility response to alarms, including 3/5/16 and 3/7/16. According to designated <br /> operator monthly inspection reports, the facility has responded to at least one alarm by removing water from a sump. <br /> -Work orders for the response to the fuel and sensor out alarms on 4/27/16. According to the designated operator <br /> monthly inspection reports,the alarm was corrected by a third party. <br /> These records shall be maintained on site for at least three years. Monitoring records include: (1)date and time of all <br /> monitoring or sampling; (2)monitoring equipment calibration and maintenance records; (3)results of any visual <br /> observations; (4) results of sample analysis performed a lab or in the field; (5)logs of all readings of gauges or other <br /> monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and <br /> reconciliations.
