The training for two others, Eugenia Valdez and Alberto,was last done on 4/12/15 and is now <br /> expired. None of these three people were on site at the beginning of the inspection. The designated operator shall <br /> train facility employees for which he or she is responsible in the proper operation and maintenance of the UST system <br /> once every 12 months. The training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4. Name of the contact person(s)for emergencies and monitoring equipment alarms <br /> Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of <br /> the training records to the EHD. n_(`C.. , CA �, •i �N 1 U S N .1 J <br /> This is a repeat violation, Class 11. \' e�,v , -2-Z ) 0 - ` w �l <br /> 309 CCR 2636(f)(1) Monitoring in the UDCs failed to activate an audible and visual alarm or stop the flow at the UDC. <br /> The Bravo box float and chain in UDC 7/8 (south)sump failed to stop the flow of product at the dispenser when tested. <br /> All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of product at the <br /> dispenser when it detects a leak. The service technician adjusted the float and verified functionality. <br /> This was corrected on site. C,// <br /> This is a repeat violation, Class II. <br /> 319 HSC 25291(e)Water in secondary containment not removed, analyzed, and properly disposed of(pre-Jul 2003). <br /> Liquid was observed in the 91 STP sump. If water could enter into the secondary containment by precipitation or <br /> infiltration, it must be removed and disposed of properly. The service technician removed four gallons of liquid from the <br /> 91 STP sump.
